Physical address on form 2. Exact mileage claimed 3. Deadline for acceptance 4. Signed hard copy to county superintendent for approval 10. TR-5s and TR-6s: Individual Contract and Bus Route Reimbursement Claims a. Must be signed by board of trustees’ chair b. Signed hard copy due in county superintendent’s office by 2.15 and 5.24 11. TR-13s: Bus Inspections a. Must be completed 1st and 2nd semester; 30 days prior to each semester b. If not completed by first day of school or due date or if they fail approval, they cannot operate 12. TR-35s: Bus Driver Certificates a. Review due dates of all requirements b. Driver certificates should be signed and sent to county superintendent 13. TR-2s: Bus Rider List for each bus route a. Discussion 14. Transportation Agreements a. Ramsay – Copper Mountain in past b.
