had positive feedback at the club. He states that the security is top notch.
Wayne Griffin - 7464 N 68th St - he does promotions at the club.
Ald. Bauman states that behavior has continued despite progressive discipline. He
states there is a danger of an economic deadzone in the area because of the club.
He recommends non-renewal.

a previous business. He states that in 2008 the applicant submitted several business
plans. Parking is limited. He states the number of parking spaces listed on
applications is incorrect. He also states the security information on the 2012 plan of
operation is inaccurate. He states that attempts to negotiate with the applicant have
not been successful. He supports nonrenewal.

Officer Bodo Gajevic - He worked with the HIDTA (High Intensity Drug Trafficking
Areas) organization to begin the investigation. Investigations were triggered by
complaints of underage performers and prostitution. Initial evidence was gathered
from the club's Facebook page. Graphic photos were present on the page. Warrants
were filed for the club's two Facebook accounts. Confidential informants who had
visited the club debriefed him. Audio and video was recorded by the confidential
source. He presented a slide show of images from Facebook, a news article, and
photos of exterior of the club.
He states that the renewal application lists 40 off street parking spaces, but the
parking slab at the establishment is only large enough to fit 4 or 5 vehicles.
On July 1st, he waited outside the establishment while the confidential source was
inside. As the club closed, he observed heavy traffic and individuals running with a
gun. He met with the applicant's husband, Douglas Gray, to explain that there was a
shooting outside and he needed video. He states that the Douglas Gray said the
shooter was never in the tavern. Afterwards, he met up with the confidential source
to obtain video. Both Officer Gajevic and Captain Smith noted that they were unable
to meet with the licensee, despite attempts and requests to do so, but were told by
Mr. Gray that he ran the establishment.
Officer Ashley Gryzkewicz - On March 26, 2013 she responded to the location based
on traffic and cruising complaints. She observed cars driving on the grass and over
the curb. On March 31, she responded to shots fired complaints. 16 shots were
fired. She spoke with the individual who fired shots, who stated she did it to stop a
fight. She also spoke with Douglas Gray. On November 26, 2013 she responded to
an underage event at the establishment. Mr. Gray said he had applied for a permit
for an underage event with the police department, but Officer Gryzkewicz was unable
to locate any application by Mr. Gray. No underage drinking was observed, but
underage individuals were seen leaving the establishment. Alcohol was not locked
up, but covered under a blanket. She stated on another occasion she gave Douglas
Gray a copy of the city ordinance in regards to costume regulations. She states that
she has observed the area surrounding the establishment to be quiet when the club
is not open.
Amanda Bowers - 5011 N 31st St - She has been to the club once in June of 2013 at
a Butt Naked Monday event. She observed nude women performing and touching
each other at the event. She states several patrons of the bar placed money on the
women.
Detective David Lopez - He is assigned to HIDTA. He spoke with a confidential
informant on February 25th who observed a sign up sheet for strippers on the
permises, who paid $20-$40 each to perform. The informant observed nude women
who were touching each other. He also observed a semiautomatic weapon.
On March 4th, the informant observed nude women who were touching each other.
He overheard discussions about a possible Butt Naked Tuesday , as well as possible
lock-ins. On a subsequent night, the informant observed nude women and women
performing oral sex on each other. Someone also offered a gun for sale.

On June 4, the detective met with the confidential informant, who stated there were
nude women on the premises. The detective saw two people leave the
establishment. He observed one of the two people fire a shot into a nearby field.
Detective Phil Simmert - Assigned to HIDTA. On February 25, 2013, a confidential
informant stated drugs were for sale on the premises and sexual acts were observed.
On March 4, 2013 the detective observed gridlocked traffic and smelled burnt
marijuana.

Douglas Gray - He stands by the statements that he made in his written response
(exhibit #1). He said that there are no guns or illegal drugs on the premises and was
unaware of any stripping taking place at his establishment, but concedes it did take
place due to the photographic evidence. He primarily stays in the front of the
business while the dance floor is in the back. Since the business has opened, they
have made many improvements to the building. He made complaints to the police
department about a nuisance property next door to the establishment and later
purchased the building. He states that he has participated in negotiations with the
police department and city attorney's office. He states that they will surrender the
stripper license and accept punishment.
Ald. Kovac moved to accept withdrawal of all requested changes and removal of the
strippers on the Public Entertainment Premises license application (Prevailed 5-0)
Wayne Carter - 2459 N 39th St - He states he knows Douglas Gray and has visited
the establishment several times. He has watched tv and played pool in the
establishment in the day time. He has not witnessed any illegal activity.
Homer Blow - 2003 W Capitol Dr - He states he has visited the establishment several
times. He did not observe any drug activity.
Kevin Jones - 926 W Center St - He has worked as a DJ in the establishment. He
was present for some "BNM" event nights. He has not observed illegal activity.

not in favor of the Public Entertainment Premises license.

Ald. Hines at the table - States Douglas Gray has been engaged in high school
attendance meetings at the YMCA. Mr. Gray reached out to the alderman early in
2013, requesting a meeting with the police, but the meeting could never be
organized. He states that he is disappointed that the police did not work with his
office to correct the issues.
Ald Coggs moves to accept withdrawal of the Permanent Extension of Premises

Ald. Dudzik moved nonrenewal based on neighborhood testimony, police testimony,
