The water line has been installed up to the north side of the building. 2.1.5. The force main has been bored in place, up to the Lift Station. 2.1.6. Change Order #01 – Value Engineering: Approved 9. Applications for Progress Payments 9.1. Pay Application No: 001 – approved 06/02/2022. 9.1.1. Distributed copies during the meeting. 9.1.2. Angela noted that she needs to review and approve before the pay application is given to the Owner. 9.1.3. The Contractor submitted the pay application using the AIA Document G702 and 703. Angela questioned the use of the AIA document and will review. 9.1.3.1. After review, Angela indicated that per the Pre-Construction Meeting Minutes, the Contractor shall submit their pay application using the Rural Development Form FmHa 1924-18. 9.1.3.2. The Contractor was directed later to re-submit Pay Application #1 on the Rural Development Form. 10. Schedule next Progress Meeting: 10.1. The next progress meeting is scheduled for June 16th. 10.1.1. Meet at City Hall; Time: 10:30 AM. 10.1.2. Angela noted that she is planning to attend progress meetings once a month and will not be able to attend the next meeting. 11. Other Discussion Items: 11.1. The Contractor was directed to provide an up-dated construction schedule for each progress meeting. Page 4 of 25  11.2. Angela indicated that there maybe a Rural Development form to use for the site observation reports. She will review and notify. 11.2.1. After review, there isn’t a Rural Development Form to use for the site observation reports. In addition, the site observation reports can be attached to the progress meeting minutes. If in error, omission, or in need of clarification, contact WiDSETH or mention the issue at the next meeting. Range: 60 - 62 Contractors on site: Footing crew, lift station crew and site crew Observation: 1. Project’s construction sign is installed. 2. Silt fencing has been installed as indicated on the drawings. 3. The contractor was installing the lift station. 4. Granular fill was being hauled to the site. Three loads were delivered while I was on site. A bulldozer was spreading the granular fill. 5. Below the sub-cutting of the existing grade at the building area, an additional layer of organic/peat soils could be observed at the west side and a portion of the north and south sides of the building perimeter. 6. The footings had been installed. The contractor used the circular option on 95% of the footings. a. The contractor indicated that a couple of the footings at the west side required a rock bed be installed as indicated in the Geotechnical Evaluation below the footing pad. 7. The contractor had started removing the footing forms. 8. The footing trench had water in it from the rain this past weekend and the contractor was pumping it out. 9. The wood roof trusses are on site. 10. The wood columns were being unloaded at the construction site. 11. Photos were taken of the entire site and building area. Information or Action Required: Address the organic/peat soil layer at the building area. Status of Proposal Requests: 7.1. None currently. 8. Status of Change Orders: 8.1. Change Order #01 – Value Engineering: Approved 8.2. Change Order #02 – bad soils at building area: Approved June 16, 2022. 8.2.1. WiDSETH to forward a sign copy to the Contractor. 9. Applications for Progress Payments 9.1. Pay Application No: 001 – was approved. 9.2. The next pay application is scheduled to be submitted at the end of the month. 10. Schedule next Progress Meeting: 10.1. The Contractor requested the Owner to complete the application for power at the construction site. 11.1.1. The Owner has the application completed with Beltrami Electric. 11.2. Mike indicated concern with the finish elevation of the lift station being too low. 11.2.1. After review, once the finishing grading is complete there is no issue with the lift station elevation. 11.3. Mike indicated concern with the elevation of the finish grade at the SE road access. 11.3.1. After review, the Contractor isn’t complete with the finish grading. 11.3.1.1. In addition, WiDSETH to review the plan for the finish paving at the SE road access. 11.4. Mike indicated concern with SW road access design. 11.4.1. WiDSETH to review alternate solution for the grading at the SW road access by the storm pond overflow. If in error, omission, or in need of clarification, contact WiDSETH or mention the issue at the next meeting. Range: 62 - 64 Contractors on site: Framing crew Observation: 1. The sites rough grading is complete. 2. The grading at the SE road access needs to be corrected. 3. The building columns are installed and the columns at both gable end, extend up into the attic area as noted. 4. The exterior wall girts are installed. 5. The LVL beams are currently being installed. The beams at the north and east walls are in place and the Contractor is working on the west wall. a. Contractor to verify to follow ASI #1 fastening instructions. 6. The LVL for the future “lean-to” is in place. a. The Contractor to verify that it is a wolminized LVL. 7. The building’s sub-grade is built up and the gravel at the site’s paving area is installed. 8. The building’s perimeter below grade rigid insulation is installed. The vertical section was visible. 9. Photos were taken of the entire site and building area. Information or Action Required: 1. Contractor to verify to follow the fastening instructions of ASI #1. 2.
