year and they will be donating to local food pantry; Happy Thanksgiving.

information.

State Statutes require that every even numbered year new terms begin and the Mayor of
each municipality shall nominate to the governing body election workers for each voting
district to be approved before December 31 of the odd number years. All applicants for
election official must meet the following qualifications:

It was time for an update on language to the City of Columbus Open Shelter
applications as well our Rest Haven application. In your packets are updated versions
created with the help from Attorney Johnson.
I created new fillable forms that are user friendly for our customers. Each form is a
Microsoft word document formatted and locked to allow data entry.
The new forms not only look good, but also provide a seamless experience for our
customers.
I have also created a pdf fillable version to our Pavilion contract.

The person / group named in this application will be responsible for the conduct of the group and for the condi-
tion of the reserved building area. This permit is subject to all Municipal Ordinances as defined by the City of
Columbus, in addition to all rules and regulations governing the City's Park division. The applicant agrees that
during the use of the facility, the sponsoring organization will not exclude anyone from participation in, deny
anyone the benefits of or otherwise subject anyone to discrimination because of the person’s race, color,
creed, national origin or handicap. Park hours are from 6am—10pm. Alcoholic beverages are allowed in the
building, no glass bottles allowed. If the alcohol is provided as complimentary then a picnic license is not nec-
essary; should there be a fee for the alcohol, a picnic license IS required. Please inquire with Amy Jo Meyers
of the Recreation Department.
The applicant has read the Open shelter Policies. The applicant has included all of the appropriate permit appli-
cations and materials for this event (if needed). The applicant agrees to remove all garbage from the premises
and place in the dumpsters provided by the parking lot and agrees to clean the shelter and leave as it was
found. Failure to do so may result in loss of deposit.

A refundable deposit is required along with your Rest Haven application. The deposit amount is the amount of a full day rental
dependent on residency and nonprofit status. Applications must be filled completely to guarantee your request. If your
reservation is requested less than 60 days from the date of your requested reservation, payment must be made in full. If your
reservation request is made more than 60 days from the date of your requested reservation, the balance payment of rent and
Wisconsin sales tax is due 60 days from your date of request. Full payment requires payment of rent, Wisconsin sales tax and
deposit.
The City of Columbus does not offer half-day rentals on the Rest Haven. Available to rent Monday - Sunday, Holiday rates are the
same as weekend rates. Should the venue be cleaned and free of damage, the City of Columbus will return your deposit to you in the
form of a check from the City of Columbus. The City of Columbus will cash your deposit check. **Nonprofit rates with proof of

Rest Haven Rules
1. RESERVATIONS:
a. Reservations are granted to persons age 18 & over only.
b. Reservations are accepted on a first-come, first-served basis and must be made a minimum of 7 days in advance. No
use of the shelter is allowed without an approved reservation.
c. Rest Haven shelter can be reserved May 1 - Mid-October (weather dependent), annually.
2. FEES: All fees are payable in advance. If a reservation is cancelled less than 30 days before the event, no refund will be
granted. If the reservation is canceled more than 30 days before the event, the City of Columbus will deduct a $25
administration fee from your deposit and / or payment of rent and Wisconsin sales tax and will return the balance.
3. RESERVATION CHANGES: The shelter or date reserved can be changed prior to your event, depending upon availability,
by contacting the Columbus Recreation office. There will be a $5.00 service fee assessed each time a change is performed.
4. KEYS:
a. Keys can be picked up from the Columbus Recreation Department office as early as 2 business days prior to the
reservation date.
b. Keys shall be returned to the Columbus Recreation Department following the event. They can be placed in the drop
box located at 229 E School Street or returned in person during office hours within 48 hours following your event.
c. There is a $100.00/ key replacement fee for all keys not returned.
5. PARK HOURS: City parks are open 7 days per week, 6:00am-10:00pm. You are not allowed in the park prior to or after
these published hours.
6. TENTS/AIR SUPPORTED STRUCTURES (BOUNCE HOUSES)/STAKES: Anything to be driven into the ground requires
contact with Diggers Hotline by calling 811 for line location 3 business working days prior to your event. The City of Columbus
will not call Diggers Hotline for you. Failure to do so may result in full deposit loss.
7. CLEAN-UP SUPPLIES: Cleaning supplies are included w/ the Rest Haven rental. All debris must be placed in the waste
receptacles upon conclusion of the event. If waste receptacles are full, place extra debris in a garbage bag and leave closed
garbage bags next to waste receptacles. Do not put hot coals from grilling in the dumpsters or garbage bags. You are responsible
leaving the shelter in a clean, damage free condition upon the conclusion of the event. Any clean-up and/or repair costs will be
assessed to the permit signer.
8. SHELTER/BUILDING APPEARANCE: No changes in the physical appearance of a shelter/building shall occur, including
placing nails or staples in the woodwork or picnic tables. Damages will be assessed to the permit signer.
9. PICNIC TABLES: Some picnic tables are available. Additional tables can be requested for a fee of $10 each.
10. GRILLING: Grills are not supplied at the open shelter, you may supply your own. Grilling is not allowed underneath or
within 20' of a shelter or tent. If using a charcoal girl, when done grilling, let coals burn down. Do not dispose of hot coals in
the dumpsters or garbage bags. Bring a water source with you to cool coals down per the Columbus Fire Department. Water
source must be within 5 feet of the grill.
11. ELECTRICAL OUTLETS: Do not overload the electrical outlets. We suggest bringing food in slow cookers/crock pots
versus using Nesco roasters (maximum of 1 Nesco per outlet). If an outlet blows, press the re-set button.

A refundable $100 deposit is required along with all Shelter rental applications. Applications must be filled completely to guarantee your request. If your reservation
is requested less than 60 days from the date of your requested reservation, payment must be made in full. Full payment requires payment of rent, Wisconsin sales tax
and deposit. If your reservation request is made more than 60 days from the date of your requested reservation, the balance payment of rent and Wisconsin sales tax
is due 60 days from your date of request.”
A Full day rental for each Open Shelter and / or Concession stand area is $50.00 plus Wisconsin Sales Tax of $2.75.
The City of Columbus does not offer half-day rentals on Open Shelters or Concession stands. Monday - Sunday, and Holiday rates are the same as weekend rates.
Should the venue be cleaned and free of damage following use, the City of Columbus will return your deposit to you in the form of a check from the City of Columbus.
The City of Columbus will cash your deposit check.
**Nonprofits with proof of 501c3 status will be tax exempt.
**Nonprofits will receive a 50% reduction on a full day rental price and a full deposit is required of $100.

Open Shelter Reservation Policies
1. RESERVATIONS:
a. Reservations are granted to persons age 18 & over only.
b. Reservations are accepted on a first-come, first-served basis and must be made a minimum of 7 days in advance.
c. Open air shelters can be reserved May 1 - Mid-October, annually. Dates requested beyond these dates are
dependent on the weather, there is no guarantee that the restrooms are available for use.
2. FEES: All fees are payable in advance. If a reservation is cancelled less than 30 days before the event, no refund will
be granted. If the reservation is canceled more than 30 days before the event, the City of Columbus will deduct a $25
administration fee from your deposit and / or payment of rent and Wisconsin sales tax and will return the balance.
3. RESERVATION CHANGES: The shelter or date reserved can be changed prior to your event, depending upon
availability, by contacting the Columbus Recreation office.
There will be a $5.00 service fee assessed each time a change is performed.
4. KEYS:
a. Keys can be picked up from the Columbus Recreation Department office as early as 2 business days prior to the
reservation date.
b. Keys shall be returned to the Columbus Recreation Department following the event. They can be placed in the
drop box located at 229 E School Street or returned in person during office hours within 48 hours following your
event.
c. There is a $100.00/key replacement fee for all keys not returned.
5. ON THE DAY OF YOUR EVENT: Take the permit and receipt to the shelter. Shelters are reserved to 1 party per day.
If someone is using the shelter upon your arrival, politely tell them that you have it reserved, show them your permit and
receipt and ask them to leave. If they refuse to leave, notify the Police Department at 623-5919 immediately.
6. PARK HOURS: City parks are open 7 days per week, 6:00am-10:00pm. You are not allowed in the park prior to or
after these published hours.
7. TENTS/AIR SUPPORTED STRUCTURES (BOUNCE HOUSES)/STAKES: Anything to be driven into the ground
requires contact with Diggers Hotline by calling 811 for line location 3 business working days prior to your event. The City
of Columbus will not call Diggers Hotline for you. Failure to do so may result in full deposit loss.
8. CLEAN-UP SUPPLIES: For rental of an Outdoor shelter, bring a broom, dustpan, cleaning supplies, sturdy see-
through garbage bags, etc. to clean the shelter area before leaving. All debris must be placed in the waste receptacles upon
conclusion of the event. If waste receptacles are full, place extra debris in a garbage bag and leave closed garbage bags next to
waste receptacles. Do not put hot coals from grilling in the dumpsters or garbage bags. You are responsible leaving the
shelter in a clean, damage free condition upon the conclusion of the event. Any clean-up and/or repair costs will be
assessed to the permit signer.
9. SHELTER/BUILDING APPEARANCE: No changes in the physical appearance of a shelter/building shall occur,
including placing nails or staples in the woodwork or picnic tables. Damages will be assessed to the permit signer.
10. PICNIC TABLES: Picnic Tables are available. If the tables are moved, they must be placed in their original place or
costs of $1o per table will be assessed to the permit signer.
11. GRILLING: Grills are not supplied at the open shelter, you may supply your own. Grilling is not allowed underneath
or within 20' of a shelter or tent. If using a charcoal girl, when done grilling, let coals burn down. Do not dispose of hot
coals in the dumpsters or garbage bags. Bring a water source with you to cool coals down per the Columbus Fire
Department. Water source must be within 5 feet of the grill.
12. ELECTRICAL OUTLETS: Do not overload the electrical outlets. We suggest bringing food in slow cookers/crockpots
versus using Nesco roasters (maximum of 1 Nesco per
outlet). If an outlet blows, press the re-set button.
