Resolution Recognizing Public Power Week October 1-7, 2023 and How
Columbus Utilities Makes Our Lives Powerful All Year
WHEREAS, we, the citizens of Columbus, place a high value on local control over community services and therefore have
chosen to operate a community-owned, locally controlled, not-for-profit electric utility and, as consumers and owners of our
electric utility, have a direct say in utility operations and policies; and
WHEREAS, Columbus Utilities has provided our homes, businesses, farms, social service, and local government agencies
with reliable, efficient, and cost-effective electricity for over 100 years, employing sound business practices designed to
ensure the best possible service at not-for-profit rates; and
WHEREAS, Columbus Utilities is a valuable community asset that contributes substantially to the well-being of local
citizens through energy efficiency, customer service, environmental protection, economic development, and safety awareness;
and;
WHEREAS, Columbus Utilities is a dependable and trustworthy institution whose local operation provides many consumer
protections and continues to make our community a better place in which to live and work, as well as contributes to protecting
the global environment;
NOW, THEREFORE BE IT RESOLVED: that Columbus Utilities will continue to work to bring lower-cost, safe, reliable
electricity to community homes and businesses just as it has since 1898, the year when the utility began to serve all the
citizens of Columbus; and
BE IT FURTHER RESOLVED: that the week of October 1-7, 2023, be designated Public Power Week in order to honor
Columbus Utilities for its contributions to the community and to make its consumer-owners, policy makers, and employees
more aware of Columbus Utilities overall contributions to their well-being and how it makes their lives powerful: and
BE IT FURTHER RESOLVED: that our community joins hands with more than 2,000 other public power systems in the
United States in this celebration of public power recognition that Columbus Utilities is good for consumers, business, the

Fire Prevention Week – October 8 – 14, 2023
WHEREAS: Joe Hammer, City of Columbus Mayor has proclaimed Sunday, October 8th
through Saturday, October 14th, 2023, as Fire Prevention Week, and urges all the people of the
City of Columbus to check their kitchens for fire hazards and use safe cooking practices during
Fire Prevention Week 2023, and to support the many public safety activities and efforts of the
Columbus Fire Department; and
WHEREAS: The City of Columbus is committed to ensuring the safety and security of all those
living in and visiting our city; and
WHEREAS: Fire is a serious public safety concern both locally and nationally, and homes are the
locations where people are at greatest risk from fire. Home fires killed more than 2,800 people in
the United States in 2021, according to the National Fire Protection Association® (NFPA®), and
fire departments in the United States responded to 338,000 home fires; and
WHEREAS: Cooking is the leading cause of home fires in the United States and fire departments
responded to more than 166,400 annually between 2016 and 2020. Two of every five home fires
start in the kitchen with 31 percent of these fires resulting from unattended cooking. More than
half of reported non-fatal home cooking fire injuries occurred when the victims tried to fight the
fire themselves; and
WHEREAS: Children under five face a higher risk of non-fire burns associated with cooking than
being burned in a cooking fire. Columbus residents should turn pot handles toward the back of the
stove; always keep a lid nearby when cooking; keep a three-foot kid-free zone around the stove,
oven, and other things that could get hot; watch what they heat; and set a timer to remind them that
they are cooking; and
WHEREAS: Residents who have planned and practiced a home fire escape plan are more
prepared and will therefore be more likely to survive a fire. Working smoke alarms cut the risk of
dying in reported home fires almost in half; and
WHEREAS: Columbus Fire Department personnel are dedicated to reducing the occurrence of
home fires and home fire injuries through prevention and protection education; and
WHEREAS: Columbus residents are responsive to public education measures and can take
personal steps to increase their safety from fire, especially in their homes; and
WHEREAS: The 2023 Fire Prevention Week theme, “Cooking safety starts with YOU. Pay
attention to fire prevention,” effectively serves to remind us to stay alert and use caution when
cooking to reduce the risk of kitchen fires; and
WHEREAS: The Mayor and Council calls upon each resident and visitor of the City of Columbus
to observe Fire Prevention Week with appropriate programs and activities and to renew efforts to
prevent fire and their tragic consequences for human health and safety.
NOW, THEREFORE, BE IT PROCLAIMED, by the Mayor and City Council of the City of
Columbus, Wisconsin that the week of October 8th through October 14th, 2023, is hereby
proclaimed:

FURTHER, the Mayor and City Council of the City of Columbus, do hereby commend the
Columbus Fire Department for its outstanding efforts and service in the protection of the Residents

On September 19, 2023 the city council approved ordering a 2024 squad. The window for pricing release
and ordering were opening soon. There is an active substantial backlog of squad orders (orders placed
but not fulfilled) from both 2022 and 2023. Ford is not taking new orders for 2024 squads to clear their
backlog. Any Ford's ordered this fall will be a 2025 model year and delivered early 2025. Dealers have
been told to anticipate substantial price increases from all manufacturers, up to 15%-20% over previous
model years.
Chevrolet was due to release their pricing September 26, 2023 and I was planning to order a new
Chevrolet Tahoe PPV (police pursuit vehicle rated) as soon as pricing was released.
On September 22, 2023 I was notified by Napleton Chevrolet Columbus (who has the state contract
pricing for Chevrolet's) that due to the UAW (United Auto Workers) strike, the pricing release and
ordering have been delayed by General Motors.
On September 25, 2023 I was notified by Napleton Chevrolet that 2--2023 Chevrolet Tahoe PPV's they
had received recently for a Sheriff's department in northern Wisconsin were available as the
department had changed their mind. One was immediately sold to another agency in this county and
the 2nd one was offered to me.
The price, all inclusive of titling and registration is $41,547.94. I had budgeted $50,000 for the 2024 I was
going to order as that is where the pricing is anticipated to be.
I checked with city hall and funds are available in the general fund for this purchase in this year's budget.
That would free up $50,000 of next years proposed budget. I also budgeted $30,000 in 2024 for the flip
(stripping of car we are retiring, purchase and install of new equipment), a new radar unit and squad
video system. Both of those in the squad we are retiring are at the end of their useful life. I would leave
the funding for those in the 2024 budget as our vendor won't get to this vehicle until after the first of
the year. I will order the equipment after January 1, 2024.
In summary, I am requesting approval to purchase this 2023 Chevrolet Tahoe from Napleton Chevrolet
for $41, 547.94.

Background
CBC DT LLC has made application for a Certified Survey Map to combine two parcels for development

review.
2. The approval is contingent upon the sale of the property to CBC DT LLC. If the sale of the property
does not occur, the CSM will be considered rejected.

The Plan Commission reviewed the request at their regularly scheduled meeting on September 14, 2023.
Motion by Monday, second by Albright to recommend approval of the CSM with the conditions as noted by
staff within the report dated September 14, 2023. Motion carried unanimously.

Dear Chris:
Ruekert & Mielke, Inc. has completed our review of the certified survey map for the above referenced
project, dated August 23, 2023. This review is for the compliance with Wisconsin Administrative Code
Chapter A-E 7, Wisconsin Statute Chapter 236 utilizing the Wisconsin CSM checklist provided by plat
review, and the City of Columbus Ordinances Chapter 90, Article VI. This review does not constitute a
recommendation for future phases. This review may not include all City requirements for this submittal.
We offer the following comments, clarifications, or concerns:
General:
1. Per Wisconsin Administrative Code AE-7 – No comments.
2. Per Wisconsin Statute Chapter 236:
A. Per Wisconsin Statute Chapter 236.34 (1m) (a) – Survey performed by a WI Professional Land
Surveyor. PLS 2015 is William Rensberger, the PLS number for Steve Roberts should be 3015.
B. Per Wisconsin Statute Chapter 236.34 (1m) (c) – The location of the land by government lot,
recorded private claim, quarter-quarter section, section, township, range and county noted under
the CERTIFIED SURVEY MAP heading. The heading is missing the quarter section.
C. Per Wisconsin Statute Chapter 236.20 (2) (h)– The centerline of all streets. The centerline of W.
James Street should be shown.
D. Per Wisconsin Statute Chapter 236.20 (2)(k) – When a street is on a circular curve, the main
chords of the right-of-way lines shall be drawn as dotted or dashed lines in their proper places.
Main chord for C1 needs to be shown.
E. Wisconsin Statute Chapter 236.20 (2)(k) – Curves shall show the radius, the central angle, chord
bearing, the chord length and the arc length for each segment. The central angle information is
missing.
F. Per Wisconsin Statute Chapter 236.20 (3)(d) – The names of adjoining streets, state highways
and subdivisions shown in their proper location underscored by a dotted or dashed line.
Other:
1. Provide and label document # that provides the 60-foot-wide access to STH 16/60 as noted on the
CSM.

7. 90-212 (4) - All existing structures, together with an identification of the type of structure, such as
residence, garage, barn, or shed; the distances of such structures from existing and proposed
property lines, wells, watercourses, and drainage ditches; and existing property boundary lines in
the area adjacent to the exterior boundaries of the proposed certified survey map and within 100
feet thereof.
8. 90-212 (6) – Building or setback lines required.
9. 90-212 (7) - The owners of record of abutting unplatted lands shall also be shown.
10. 90-212 (8) - Length and bearing of the centerline of all streets. The lengths shall be given to the
nearest 0.01 foot and the bearings to the nearest one second of arc. The arc, chord, and radius
lengths, and the chord bearing, together with the bearings of the radii of the ends of the arcs and
chords, shall be given for all curved lines.
11. 90-215 - The surveyor shall certify that he or she has fully complied with all of the provisions of this
chapter.
12. Create and list proposed easements required for new development:
A. City of Columbus Utility will require a new proposed 15-foot electrical easement that should run
the full length of the western property line. A separate easement document shall be required that
outlines responsibilities and rights of the easement.
B. City of Columbus Utility will require a storm water easement (size to be determined) to be
provided on the south property line and western property line for purposes of conveying private
and public storm water as required. A separate easement document shall be required that
outlines responsibilities and rights of the easement.

A report was completed by JSD for the Boy Scout Cabin at Fireman’s Park. The report
was dated October 17, 2022. Based upon that report, a discussion by Council in
summer 2023 resulted in a request for an additional scope of work to be provided by
JSD for structural engineering consulting services to further look at the existing decay
within the building.
The additional cost for this work is $2,400.

Dear Ms. Wilson:
Thank you for considering JSD Professional Services, Inc. (JSD) to provide professional consulting
services for the above-described project. We are pleased to submit this proposal and look forward
to working with you on this project.
SCOPE OF WORK
Our understanding of the scope of work is based upon preliminary conversations with Mr. Jerry
Lentz, as it relates to the structural inspection of the existing scout cabin located in Fireman’s Park
in Columbus, Wisconsin. JSD will provide professional structural engineering consulting services for
the inspection of the cabin to address the existing decay within the timber framing.
The scope of work for these items is described in the following tasks:
STRUCTURAL ENGINEERING SERVICES
Task 1.0: Conditional Evaluation Report
JSD will perform the following:
• Perform a site inspection of the existing cabin timbers to evaluate the extent of wood decay
within the exterior load bearing wall timbers.
• Prepare a report evaluating the extent of decay within the exterior wall timbers and
prepare two (2) options for remediation and replacement of compromised timbers.
• Prepare a cost estimate for each reconstruction/repair option provided.
• Inspect and evaluate the existing floor framing of the structure. The inspection will include
evaluation of the crawlspace and floor framing.
• Inspect the existing roof framing and document deficiencies and required repairs.

GENERAL PROJECT UNDERSTANDING
1. The existing building is a single-story log-framed timber building that is comprised of two (2)
timber buildings connected together.
2. The existing building sits on a cast-in-place concrete foundation with a crawlspace.
3. Existing timbers at the base of the cabin have experienced significant decay and wood rot.
4. The City of Columbus wants to understand the overall condition of the structure and
determine the extent of repairs require to keep the facility in a safe and usable condition for
the extended future.
COSTS
Based on our understanding of the scope of work and the above-described task, we will perform
the professional services for a lump sum fee, as follows:
Task 1.0: Conditional Evaluation Report $ 2,400.00
This cost estimate was developed based upon our understanding of the project as described in this
proposal. All contract amendments, and any increase in costs or fees over the amounts set forth
above, must be pre-approved by the Client in writing.
AUTHORIZATION
Our services will be performed in accordance with the enclosed JSD Agreement for Professional
Services upon acceptance of this proposal. Additional scope of services not included in this
proposal will be agreed-upon by both parties, in writing, prior to proceeding. Invoice disputes must
be made within 30 days of date of invoice.
Please indicate your acceptance of this proposal by signing and returning a copy of the attached
JSD Agreement for Professional Services to confirm our understanding of your needs and authorize

Agreement for Professional Services
Scout Cabin Inspection, Columbus, WI - JSD Project #22-11938
JSD Professional Services, Inc. (Consultant) will perform professional services for City of Columbus (Client), according to the terms and
conditions of this Agreement. The Consultant shall perform the services outlined in the attached Proposal dated September 19, 2023, in
consideration of the stated fee and payment terms. For additional services not included above, the Consultant shall be compensated
according to hourly rates agreed to between Consultant and Client, or as specified in the Proposal referenced above.
Terms and Conditions
1. Access to Site: Unless otherwise stated, the Consultant will have access to the site for activities necessary for the performance
of the services. The Consultant will take reasonable precautions to minimize damage due to these activities, but has not
included in the fee the cost of restoration of any resulting damage and will not be responsible for such costs.
2. Retainer, Billing, and Payment: The Client agrees to pay the Consultant for all services performed and all costs incurred, as
described in the Proposal attached to this Agreement. Prior to providing services, the Client shall deposit a retainer of
$ 0 with the Consultant. Invoices for the Consultant's services shall be submitted, at the Consultant's option, either upon
completion of such services or on a monthly basis. Invoices shall be due and payable upon receipt. For any invoice not paid
within 60 days, the Consultant may, without waiving any claim or right against the Client, and without liability whatsoever to
the Client suspend or terminate the performance of services. The retainer shall be credited on the final invoice. Accounts
unpaid 30 days after the invoice date may be subject to a monthly service charge of 1.5% (or the maximum legal rate) on the
unpaid balance. In the event any portion of the account remains unpaid 90 days after the billing, the Consultant may institute
collection action and the Client shall pay all costs of collection, including reasonable attorney's fees. All invoice inquiries must
be made within 30 days of invoice date.
3. Indemnification: The Client shall, to the fullest extent permitted by law, indemnify and hold harmless the Consultant and
subconsultants from and against all damage, liability and cost, including reasonable attorneys' fees and defense costs, arising
out of or in any way connected with the performance of the services under this Agreement.
The Consultant agrees, to the fullest extent permitted by law, to indemnify and hold harmless the Client, its officers, directors
and employees (Client) against damages, liabilities or costs, including reasonable attorneys' fees and defense costs, to the
extent caused by the Consultant’s negligent performance and that of its subconsultants or anyone for whom the Consultant
is legally liable.
Neither the Client nor the Consultant shall be obligated to indemnify the other party in any way whatsoever for the other
party's own negligence.
4. Information for the Sole Use and Benefit of the Client: All opinions and conclusions of the Consultant, whether written or
oral, and any plans, specifications or other documents and services provided by the Consultant are for the sole use and
benefit of the Client and are not to be provided to any other person or entity without the prior written consent of the
Consultant. Nothing contained in this Agreement shall create a contractual relationship with or a cause of action in favor of
a third party against either the Consultant or the Client.
5. Certifications, Guarantees and Warranties: The Consultant shall not be required to execute any document that would result
in the Consultant certifying, guaranteeing or warranting the existence of any conditions. The Consultant will render services
and opinions according to the standards of practice of the applicable consulting professional engineering and surveying.
6. Lien Rights: Consultant hereby notifies the Client/Owner that persons or companies performing, furnishing or procuring
labor, services, materials, plans or specifications for the construction on Client’s/Owner’s land may have lien rights on
Client’s/Owner’s land and buildings if not paid in accordance with the construction lien law in the state in which the project
is located.
7. Standard of Care: In providing services under this Agreement, the Consultant shall perform in a manner consistent with and
limited to that degree of care and skill ordinarily exercised by members of the same profession currently practicing under
similar circumstances at the same time and in the same or similar locality. The Consultant makes no warranty, express or
implied, as to its professional services rendered under this Agreement.
8. Risk Allocation: In recognition of the relative risks, rewards and benefits of the project to both the Client and the Consultant,
the risks have been allocated such that the Client agrees that, to the fullest extent permitted by law, the Consultant's total
liability to the Client for any and all injuries, damages, claims, losses or expenses arising out of this Agreement from any cause

or causes, shall not exceed $100,000.00 or the Consultant’s total fee for services rendered for this project, whichever is
greater. Such causes include, but are not limited to, Consultant's negligence, errors, omissions, strict liability, breach of
contract or breach of warranty.
9. Ownership of Documents: All documents produced by the Consultant under this Agreement are instruments of the
Consultant's professional service for use in the project for which the Consultant was retained. These documents may not be
used by the Client for any other purpose without the prior written consent of the Consultant. Any electronic files transferred
by the Consultant are provided solely for the convenience of the Client and are transmitted in trust for the sole use of the
Client and its acceptance constitutes assumption of responsibility for its use and safekeeping. Any use by third parties shall
be at the sole risk of the Client. Any alterations, tampering with files or deviation in any respect from the original content
shall render the entire contents void and shall constitute the agreement of the Client to release, defend and hold harmless
the Consultant, from all claims and causes of action by Client and third parties.
10. Photographic and Videographic Consent: Consultant shall have the right to include photographic, videographic and artistic
representations of the Project, generated and/or directed by Consultant, among Consultant’s promotional and professional
materials. Consultant shall be given reasonable access to the Project Site to create such representations. However,
Consultant’s materials shall not include the Client’s/Owner’s confidential or proprietary information if the Client/Owner has
previously advised Consultant in writing of the specific information considered by the Client/Owner to be confidential or
proprietary. If the representations generated by and/or directed by Consultant are used in or for promotional and
professional materials by others, the user(s) shall provide professional credit to Consultant in those materials. This Section
shall survive the termination of this Agreement unless the Client/Owner terminates this Agreement for cause pursuant to
Section 11 of JSD’s Agreement for Professional Services.
11. Dispute Resolution: With regard to any claims or disputes between the Client and the Consultant arising out of the services
to be provided by the Consultant or out of this Agreement, the parties agree to attempt to resolve such disputes through
direct negotiations between the appropriate representatives of each party. If such negotiations are not fully successful, the
parties agree to attempt to resolve any remaining disputes by formal nonbinding mediation conducted in accordance with
rules and procedures to be agreed upon by the parties.
12. Termination of Services: This Agreement may be terminated at any time by either party should the other party fail to perform
its obligations hereunder. In the event of termination for any reason whatsoever, the Client shall pay the Consultant for all
services rendered to the date of termination, and all reimbursable expenses incurred prior to termination and reasonable
termination expenses incurred as the result of termination.
