APPROVED MINUTES RETIREMENT BOARD MEETING Tuesday, April 7, 2015 City Hall Oak Room 6:00 p.m. 1. Call to Order: The meeting was called to order by Chair Connie Rydberg at 6:10 p.m. 2. Roll Call Present: Connie Rydberg, Richard Campbell, Larry Little, Ryan Davis, Larry Little, and Jason Widstrom Absent: Elizabeth Hansen, Charles Colliers Staff Present: Richard Parker 3. Review Disability Retirement Request. Mr. Parker went over the disability retirement request from Gary Wade, former Sr. Maintenance Worker in Public Works. Mr. Wade was an employee of the City from June 13, 2015 until February 6, 2015. This is a Service Related Disability request due to Mr. Wade’s Workers’ Compensation claim on August 24, 2012. He has had three surgeries due to that injury and was unable to work from December 2013 until the City terminated his employment on February 6, 2015. The Board reviewed several physician reports from Commonwealth Orthopaedics. The Board also reviewed Section 2.28 of the Basic Pension Plan which defines Total and Permanent Disability as a physical or mental condition of a Participant which renders him incapable of continuing in the employment of the Employer and which may be expected to result in death or be of long and indefinite duration. The Board also reviewed Section 11.03 of the Plan, Service Related Disability Retirement. Mr. Parker explained that per Section 11.03 (e), Mr. Wade will not receive a disability payment from the City’s Pension Plan, if approved by the Board, until the Worker Compensation payments stop as any service related disability payments are offset by worker compensation payments and both are 66 2/3% of his salary. He would be eligible for health insurance with the City as a retiree and would accrue service time in the Basic Plan, if his request is approved. The Board also reviewed Section 11.04, Medical Examination of Disabled Participants. After considerable discussion, Ryan Davis made a motion to approve a service related disability retirement for Gary Wade under conditions spelled out in Section 11.04 of the Plan (includes medical exam in May 2016) and a copy of financial documentation starting next year. The motion passed, 4-1 (Mr.  Widstrom voted no). Mr. Wade will be retired under a service related disability retirement effective May 1, 2015. 4. Other business not on the agenda: None 5. Adjournment: Mr. Davis made a motion to adjourn. It was seconded by Vice Chair Campbell and approved, 5-0.
