BOARD OF PUBLIC WORKS AND SAFETY MINUTES January 12, 2016 Regular Session: The Board of Public Works and Safety met in regular session on Tuesday, January 12, 2016 at 9:00 a.m. in the Common Council Chambers. Members present were: Cindy Murray, Dawn Ross, Norm Childress and Ron Shriner. Jacque Chosnek, 1st Deputy City Attorney, was also present. Mrs. Murray called the meeting to order. The Pledge of Allegiance was given to the flag of our Country. MINUTES Mr. Childress moved for approval of the minutes from the January 5, 2016 regular meeting. Mr. Shriner seconded. Passed. NEW BUSINESS Engineering Permission to Advertise-Veterans Memorial Parkway & Fastline Drive Signalization Jenny Leshney, Public Works Director, presented to the Board and recommended approval of a Permission to Advertise for the Veterans Memorial Parkway & Fastline Drive Signalization near the entrance to Subaru. Mr. Shriner moved for approval. Mrs. Ross seconded. Passed. Supplement #5-LPA Consulting Contract for S18th Street Road Expansion between VMP and 430 South Mrs. Leshney presented to the Board and recommended approval of Supplement #5 for the Consulting Contract with TBird Deigns for the S 18th Street Road Expansion Project between VMP and 430 South in the amount of $25,000.00. Mrs. Ross moved for approval. Mr. Childress seconded. Passed. *Gary Henriott arrived to the meeting at 9:02a.m. Notice to Proceed-2015 CDBG Sidewalk Replacement Project Mrs. Leshney presented to the Board and recommended approval of a Notice to Proceed for the 2015 CDBG Sidewalk Replacement Project with CC&T Construction. Mr. Henriott moved for approval. Mr. Childress seconded. Passed. Agreement with TBird Designs for Parking Lot Striping Package for Lafayette City Parks Department Mrs. Leshney presented to the Board and recommended approval of an Agreement with TBird Designs for Parking Lot Striping Package for Lafayette City Parks Department in the amount of $39,990.00. This agreement allows TBird to design the ADA markings and parking lot markings for 30 parking lots in the Parks Department. Mrs. Ross moved for approval. Mr. Childress seconded. Passed. Board of Public Works and Safety January 12, 2016 Page 2 Purchasing Declaration of Worthless Property-Street Department Rick Morrissey, Purchasing Manager, presented to the Board and recommended approval of a Declaration of Worthless Property for the Street Department that includes vehicle: Veh # Year Make VIN 2033 2000 International 4900 Dump Truck 1HTSHAAR2YH265443 Mr. Morrissey stated that this vehicle is out of service and additional repairs would exceed the value of the vehicle. Mr. Shriner moved for approval. Mr. Henriott seconded. Passed. Permission to Advertise-Sodium Hypochlorite, Sodium Bisulfite, Phosphorus Chemicals & Hydrochloric Acid Mr. Morrissey presented to the Board and recommended approval of a Permission to Advertise for Sodium Hypochlorite, Sodium Bisulfite, Phosphorus & Hydrochloric Acid Chemical Bid. The chemicals will be used by the Water Pollution Control, Water Works and Parks Departments. Mr. Henriott moved for approval. Mr. Shriner seconded. Passed. Permission to Participate-Sodium Hypochlorite, Sodium Bisulfite, Phosphorus Chemicals & Hydrochloric Acid Mr. Morrissey presented to the Board and recommended approval of a Permission to Participate with the City of West Lafayette for Sodium Hypochlorite, Sodium Bisulfite, Phosphorus Chemicals & Hydrochloric Acid Chemical Bid. This will allow West Lafayette to piggyback off of the City of Lafayette’s sealed bid process. Mrs. Ross moved for approval. Mr. Childress seconded. Passed. Water Pollution Control Certificate of Substantial Completion-Earl Avenue/Durkees Run Sewer Rehabilitation Phase 4 Brad Talley, Water Pollution Control Superintendent, presented to the Board and recommended approval of a Certificate of Substantial Completion for the Earl Avenue/Durkees Run Sewer Rehabilitation Phase 4 Project. The substantial completion was complete on August 31, 2015. Mr. Henriott moved for approval. Mr. Childress seconded. Passed. Certificate of Final Completion-Earl Avenue/Durkees Run Sewer Rehabilitation Phase 4 Mr. Talley presented to the Board and recommended approval of a Certificate of Final Completion for the Earl Avenue/Durkees Run Sewer Rehabilitation Phase 4 Project. The final completion was on October 31, 2015. All of this work was compete and is satisfactory. Mrs. Ross moved for approval. Mr. Childress seconded. Passed. CLAIMS Mike Jones, Controller, presented for Board approval, Claims in the amount of $3,231,205.39. Mrs. Murray asked if there were any further questions and there were none. Mr. Shriner moved for approval. Mr. Childress seconded. Passed.
