Illinois, pursuant to notice. CALL TO ORDER: The meeting was called to order by Trustee Ryan at 8:00 a.m. ROLL CALL: PRESENT: Trustees Eric Ryan, Greg Fuchs, Lee Plate, and Ken Weigand ABSENT: Trustee David Pilgard ALSO PRESENT: None PUBLIC COMMENT: There was no public comment. Approve Duty Disability Benefits - Mark J. Bajor: The Board reviewed the duty disability benefit calculation for Mark J. Bajor prepared by L&A. Firefighter Bajor had an entry date of 10107/2001, disability date of l2l3ll20l5, effective date of pension of 01/0112076,46 years of age at date of disability, l4 years 2 months of creditable service, applicable salary of $89,690.01, applicable pension percentage of 65.00%, amount of originally granted monthly pension of $4,858.21 and amount of originally granted annual pension of $58,298.52. A motion was made by Trustee Ryan and seconded by Trustee Fuchs to approve Mark J. Bajor's duty disability benefit as calculated by L&A. Motion carried by roll call vote. AYES: Trustee Ryan, Fuchs, Plate, and Weigand NAYS: None ABSENT: Trustee Pilgard The Board noted that Mark J. Bajor is due a retroactive payment in the amount of $1,888.57 for the period October 20-31,2017 due to the termination of Workers Compensation benefits. A motion was made by Trustee Fuchs and seconded by Trustee Plate to approve the retroactive payment to Mark J. Bajor in the amount of $1,888.57 as calculated by L&A, subject to Village confirmation that Mr. Bajor was fully paid by the Village for the period October l-20,2017. Motion carried by roll call vote. AYES: Trustee Ryan, Fuchs, Plate, and Weigand NAYS: None ABSENT: Trustee Pilgard Review/Approve Decision and Order - Mark J. Bajor:The Board reviewed the Decision and Order for Mark J. Bajor. A motion was made by Trustee Weigand and seconded by Trustee Plate to adopt and publish the written Decision and Order in the Mark J. Bajor disability matter. Motion canied by roll call vote. AYES: Trustee Ryan, Fuchs, Plate, and Weigand NAYS: None ABSENT: Trustee Pilgard Approve Regular Retirement Benefits - Peter S. Przybysz: The Board reviewed the regular retirement benefit calculation for Peter S. Przybysz prepared by L&A. Lieutenant Pnybysz had an entry date of 09l20ll993,retirement date of 1lll0l20l7, effective date of pension of 1l/1112017,50 years of age at date of retirement, 24years and 1 month years of creditable service, applicable salary of $110,050.98, Lake Zurich Firefighters' Pension Fund Minutes of Meeting - November 74, 2017 Page2 of4 applicable pension percentage of 60.21yo, amount of originally granted monthly pension of $5,521.65 and amount of originally granted annual pension of $66,259.80. A motion was made by Trustee Fuchs and seconded by Trustee Weigand to approve Peter S. Przybysz's regular retirement benefit as calculated by L&A. Motion carried by roll call vote. AYES: Trustee Ryan, Fuchs,Plate, and Weigand NAYS: None ABSENT: Trustee Pilgard The Board discussed having Attorney Rick Reimer prepare a Decision and Order for the regular retirement benefits of Peter S. Przybysz. A motion was made by Trustee Ryan and seconded by Trustee Fuchs to direct Attorney Reimer to prepare a Decision and Order as stated. Motion carried unanimously by voice vote. ADJOURNMENT: A motion was made by Trustee Ryan and seconded by Trustee Weigand to adjourn the meeting at 8:10 a.m. Motion carried unanimously by voice vote.
