Enactment No: RES-15-0048 4. 15-0328 Recommendation to authorize City Attorney to submit Compromise and Release for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay $85,000 in compliance with the Appeals Board Order for Gregory Brown. OCEAN BOULEVARD COUNCIL CHAMBER, 6:00 PM A motion was made to approve recommendation on the Consent Calendar. 9. 15-0332 Recommendation to authorize City Manager to execute an amendment to Agreement No. 32607 with Galls, an Aramark Company, LLC, a Long Beach-based business, to furnish, service and deliver uniforms and accessories to the Long Beach Fire Department (LBFD), consistent with Specifications No. PA-02112, to increase the contract amount by $130,000 to $340,000 for the period ending July 1, 2015, for a total three-year contract amount not to exceed $760,000, and to extend the agreement through September 30, 2015. (Citywide) A motion was made to approve recommendation on the Consent Calendar. 10. 15-0333 Recommendation to authorize City Manager to increase Blanket Purchase Order Agreement Nos. BPFD14000008 with Bound Tree Medical, LLC, of Dublin, OH, and BPFD14000009 with Life-Assist, Inc., of Rancho Cordova, CA, for emergency medical supplies, by $100,000, for a total aggregate amount not to exceed $300,000 per year, for a period of two years, with the option to renew for two additional one-year periods, at the discretion of the City Manager. (Citywide) A motion was made to approve recommendation on the Consent Calendar. 11. 15-0334 Recommendation to authorize City Manager, or designee, to execute all necessary agreements and any subsequent amendments between the City of Long Beach and the California Department of Public Health to accept funding in the amount of $34,909, for the period of March 1, 2015 through September 30, 2015 for a program specifically for Older Adult Fall Prevention. (Citywide) A motion was made to approve recommendation on the Consent Calendar. 12. 15-0335 Recommendation to authorize City Manager, or designee, to execute all necessary documents and any subsequent Page 7 of 18 CITY OF LONG BEACH TUESDAY, APRIL 21, 2015 CITY COUNCIL MINUTES 333 W. OCEAN BOULEVARD COUNCIL CHAMBER, 6:00 PM amendments between the California Department of Public Health Tuberculosis Control Branch and the City of Long Beach to receive grant funding in the amount of $207,574, plus an allotment of up to $12,492, for a total amount of $220,066, for tuberculosis prevention and control activities for the period of July 1, 2015 through June 30, 2016. (Citywide) A motion was made to approve recommendation on the Consent Calendar. 13. 15-0336 Recommendation to authorize City Manager, or designee, to execute all necessary documents, and any subsequent amendments, between the City of Long Beach and the State Water Resources Control Board to receive funding in the amount of $100,000 to provide beach water quality monitoring from July 1, 2014 through June 30, 2015. (Citywide) A motion was made to approve recommendation on the Consent Calendar. 14. 15-0337 Recommendation to authorize City Manager, or designee, to execute an amendment to the current agreement (PH-002425) between the City of Long Beach and Los Angeles County in the amount of $444,924, and any subsequent amendments, for the provision of Ambulatory Outpatient Medical Services, for the period of March 1, 2015 through February 28, 2017. (Citywide) A motion was made to approve recommendation on the Consent Calendar. 15. 15-0338 Recommendation to authorize City Manager, or designee, to execute an amendment to the current agreement (PH-002431) between the City of Long Beach and Los Angeles County in the amount of $923,514, and any subsequent amendments, to provide Human Immunodeficiency Virus (HIV)/Acquired Immune Deficiency Syndrome (AIDS) Medical Care and Coordination Services for the period of March 1, 2015 through February 28, 2017. (Citywide) A motion was made to approve recommendation on the Consent Calendar. 16. 15-0339 Recommendation to authorize City Manager to execute an Page 8 of 18 CITY OF LONG BEACH TUESDAY, APRIL 21, 2015 CITY COUNCIL MINUTES 333 W. OCEAN BOULEVARD COUNCIL CHAMBER, 6:00 PM agreement with Veriforce, LLC, for compliance management services to meet U.S. Department of Transportation Pipeline Operator Qualification Program requirements in an annual amount of $22,475. (Citywide) A motion was made to approve recommendation on the Consent Calendar. 17. 15-0340 Recommendation to adopt resolution authorizing City Manager to execute a contract with Dresser, Inc., of Houston, TX, for furnishing and delivering rotary-type gas meters and other services and parts, without advertising for bids, in an annual amount not to exceed $150,000, for a period of two years, with the option to renew for two additional one-year periods, at the discretion of the City Manager. (Citywide) A motion was made to approve recommendation and adopt Resolution No. RES-15-0050 on the Consent Calendar. This amendment will extend the term of the agreement from December 8, 2018 to June 30, 2024, provide a discount to the operating fee, and recognize a change in the name of the operator.
