FOR THE 2021 STREET REPAIR PROGRAM PHASE 2 WHEREAS, pursuant to legal advertisement duly made, sealed bids were received on April 19, 2021 for the 2021 Street Repair Program Phase 2 in accordance with specifications prepared by the City Engineer. The work generally includes asphalt pavement resurfacing and full depth concrete repairs; and WHEREAS, the bid of Specialized Construction Inc., 711 Harvard Avenue, Cuyahoga Heights, OH 44105 for the 2021 Street Repair Program Phase 2 in the amount of $653,172.50 is determined to be the lowest and best bid, therefore. The work will include: 1. Saw cutting, removal and disposal of the existing concrete sidewalk. 2. Placement of compacted #57 LS. 3. Setting forms to match the grade of existing sidewalk to the adjacent ends of the repair area. 4. Installation ofVi inch expansion joint at the adjacent ends of the repair area. 5. Placement of QCl concrete with tooled control joints and a broom finish. 6, Topsoil /Seed grass areas adjacent to the repair that have been disturbed by our work. The cost as described above is $10.11 per square foot (SF). Terrace assumes that the City of Middleburg Heights will clearly mark the areas for repair with paint prior to the work being performed. Our work will involve cutting and removing the pavement and curb around the casting, rebuild the brickwork and reset the existing casting or if needed, place a new casting as provided by the city. After the catch basin repairs. Terrace will: 1. Backfill with compacted 304 limestone. 2. Install hook bolts on 18"- 24" centers, 3. Install expansion Joint for curbs and concrete streets. 4. Place and finish concrete curbs, 5. Place and finish seven inches of base concrete within three inches of finish pavement grade for asphalt streets. 6. Place one 3" lift of asphalt to match the existing grade and seal joints. 7. Topsoil / Seed grass areas adjacent to the repair. Terrace Construction is submitting a repair cost of $2,546.00 per location based on making 23 repairs, Terrace assumes that the City of Middleburg Heights will vac clean the proposed repair locations(groups of4 catch basins) prior to the repairs being made.
