Call To Order:
Meeting was called to order at 6:05PM by Pat Ditefano.
Approval of Minutes:
Minutes for April 13, 2021. Motion by Raven Fenmore. 2nd by Yvonne Joy. Approved.

Directors Report:
Director of Health
o Kevin Elak provided a brief update regarding the COVID status for the City of
Middletown.
Division Report
o Jennifer Sparks provided an update on environmental health, housing, emergency
preparedness, and health education activities.
Old Business: none
New Business:
The four agencies that submitted grant proposals for the FY 2021-2022 Pubic Health Block Grant
had representatives in attendance to give a presentation to the board members.
o CRT Elderly Nutrition: Julie Ackerman and Gariann Chiaralla
o Cross Street Training & Academic Center Abundance App: Dr. Michael Kalinowski
o Middlesex County NAACP: Faith Jackson, Judith Omphoy, Catherine Rees
o Northern Middlesex YMCA Schwartz Men’s Residence: Lori Lodge

Motion to adjourn by Yvonne Joy. 2nd by Anthony Mangiafico. Motion Carried. Meeting adjourned
