James Gromko, Control Room Operator-NPU, for a normal retirement eff 1/3/2015. C. Pension calculation(s): 1. Monica Bernier, retired 9/14/2014, her pension is calculated for an annual amount of $15,551.06. Her application for a normal retirement to be approved at tonight’s meeting. 2. James Curtis, retired 9/20/2014, his pension is calculated for an annual amount of $49,303.45. His application for a normal retirement to be approved at tonight’s meeting. D. Application(s) for a refund and/or rollover of pension contributions: 1. Patricia Novak, beneficiary of Joseph Novak (DOD 8/21/2014), rollover in the amount of $22,198.63 in excess contributions due beneficiary. 2. Dawn Chabot-BOE, refund in the amount of $1,041.94 with interest (through 10/31/14), who terminated eff 7/26/2013. E. Notification(s) of death: 1. Patricia Welch (DOD 9/16/2014) who retired from the City on 1/15/1994, no survivor benefit due. 2. Gerard Twomey (DOD 9/22/2014), who retired from NPU on 6/5/1982, no survivor benefit due. 3. Joseph Bernardo (DOD 9/27/2014), survivor of Carmela Bernardo who retired from NPU on 8/29/1986, no further benefit due. 4. Phyllis Gorton (DOD 9/30/2014), who retired from the City on 1/7/1999, no survivor benefit due. F. Rescind application for retirement from William Morell, Chief Lineman–NPU, approved at the 5/20/2014 meeting.
