Her pension is calculated for an annual amount of $10,216.93. Her application for a normal retirement to be approved at tonight’s meeting. 2. Teresa Kuzmenko, retired 7/9/2014, payable 3/19/2016. Her pension is calculated for an annual amount of $4,795.16. Her application for a deferred retirement was approved at the 9/16/2014 meeting. 3. Albert Costa Jr, retired 11/9/2014. His pension is calculated for an annual amount of $83,747.11. His application for a normal retirement was approved at the 10/21/2014 meeting. C. Application(s) for a refund and/or rollover of pension contributions: 1. Sharon Galipeau - BOE, refund in the amount of $31,141.98 with interest (through 12/31/2014), who terminated effective 8/27/2012. D. Notification of death(s): 1. Norma Wallace-Martin (DOD 11/13/2014), who retired from the City on 6/12/2009, survivorship benefit due Edward Martin in the annual amount of $18,819.14. E. Request from Barry Ellison, Public Works Director, to extend the eligibility list for the position of Janitor for one year. The current list expires on 1/9/2015. F.
