Application(s) for retirement: 1. Aloysius Telgarsky, Jr, Heavy Equipment Operator, for a normal retirement effective 06/02/2015. 2. Joan Barber, Administrative Assistant-BOE, for a normal retirement effective 07/01/2015. 3. Roberta McLaughlin, Secretary-BOE, for a normal retirement effective 07/1/2015. B. Pension calculation(s): 1. Thomas LaFreniere, Jr, retired 05/2/2015. His pension is calculated for an annual amount of $31,595.58. His application for a normal retirement was approved at the 12/16/2014 meeting. 2. Gayle Selvidio, retired 05/2/2015. Her pension is calculated for an annual amount of $22,861.54. Her application for a normal retirement was approved at the 4/21/2015 meeting. C. Notification of pension benefit due to a Domestic Relations Order payable to Cheryl Plante effective 05/04/2015. Her pension is calculated for an annual amount of $9,621.59. Donald Allen’s pension will be reduced to $38,486.37 annually. D. Notification of death(s): 1. Jennie Blazejak (DOD 04/21/2015), who retired from the BOE on 06/29/1991. No survivor benefit due. 2. Mary Malinowsky (DOD 4/23/2015), surviving spouse of Henry Malinowsky who retired from NPU on 04/24/1985. No further benefits due. 3. John DeLucia, Sr (DOD 04/29/2015), surviving spouse of Angela DeLucia, who retired from the BOE on 11/24/1984. No further benefit due. E. Notification of survivor benefit(s): 1. Joseph Malespini in the annual amount of $3,950.13. Notification of death for Anna Malespini acknowledged at the 01/20/2015 meeting. 2. Susan Mecteau (DOD 04/14/2015) who retired from the BOE on 07/01/2009. Survivor benefit due Harold Mecteau in the annual amount of $6,462.32. F. Request from LeeAnn Gomes, Director of Social Services, to extend the eligibility list for the position of Van Driver for one year.
