Motion to table review and possible revision of the Disability Verification Form until the June 2015 meeting made at the 05/19/2015 meeting. B. Review draft updates of the Police and Fire Pension Plan Summary books. Motion to table until the June 2015 meeting made at the 05/19/2015 meeting. C. Request from Chief Scandariato, Fire Chief, to extend the eligibility list for the position of Fire Captain for one year. The current list expires on July 9, 2015. Motion made at 05/19/2015 meeting to contact Chief Scandariato to provide a reason. D. Discussion of rollover or withdrawal of pension contributions (plus interest) upon resignation or termination of employment. Motion made at 05/19/2015 meeting to contact City Counsel for legal opinion on board approval for the disbursements after separation to non-vested, non-laid-off participants. IV. New Business A. Request from Melvin Mitchell to appear before the board. B. Application(s) for retirement: 1. Carol Johnson, Kitchen Manager-BOE, for a normal retirement effective 06/19/2015. 2. Edwin Peckham, Police Officer, for a normal retirement effective 6/20/2015. 3. Mary Ann Dixon, Code Enforcement Clerk, for a normal retirement effective 10/01/2015. C. Pension calculation(s): 1. Aloysius Telgarsky, Jr, retired 06/2/2015. His pension is calculated for an annual amount of $38,363.09. His application for a normal retirement was approved at the 5/19/2015 meeting. D. Notification of death(s): 1. Regina Ukleja (DOD 05/11/2015), who retired from the City on 6/1/1991. No survivor benefit due. E.
