CALL TO ORDER Chair Cooper called the meeting to order at 5:30 p.m. 1.A ROLL CALL Present: 4 - Chair Jim Cooper, Vice Chair Jeannine Roe, Boardmember Jack Seward, and Boardmember David Threedy Excused: 1 - Boardmember John O'Neill 2. APPROVAL OF MINUTES 2.A 14-0327 Approval of December 9, 2013 LEOFF 1 Disability Board Meeting Minutes Boardmember Threedy moved, seconded by Boardmember Seward, to approve the minutes of December 9, 2013. The motion carried by the following vote: Aye: 4 - Chair Cooper, Vice Chair Roe, Boardmember Seward and Boardmember Threedy Excused: 1 - Boardmember O'Neill 3. BUSINESS ITEMS 3.A 14-0325 Case 01-14 Hearing Aids Request Boardmember Seward moved, seconded by Boardmember Threedy, to approve the payment for hearing aids from Capital ENT in the amount of $4,800. The motion carried by the following vote: Aye: 4 - Chair Cooper, Vice Chair Roe, Boardmember Seward and Boardmember Threedy Excused: 1 - Boardmember O'Neill 3.B 14-0326 Case 02-14 Orthotics Request Boardmember Seward moved, seconded by Boardmember Threedy, to approve reimbursement for orthotics up to $450.00 with the stipulation that City of Olympia Page 1 LEOFF I Disability Board Meeting Minutes April 14, 2014 the member first submit the charges to both Regence and Medicare. Once the explanations of benefits (EOBs) are received, the member will purchase the orthotics and submit the receipt along with the EOBs to the board secretary for reimbursement up to $450.00.
