1. CALL TO ORDER
Chair Cooper called the meeting to order at 5:30 p.m.

Present: 5 - Chair Jim Cooper, Vice Chair Jeannine Roe, Boardmember Russ
Gies, Boardmember Caleb Martin, and Boardmember Jack Seward

3. APPROVAL OF MINUTES
3.A 15-0140 Approval of January 12, 2015 LEOFF Disability Board Meeting Minutes
Boardmember Martin moved, seconded by Vice Chair Roe, to approve the
minutes. The motion carried unanimously.
4. BUSINESS ITEMS
4.A 15-0141 Case #01-15 Hearing Aid Request
Boardmember Seward recused himself because he is the requester for the hearing
aids.
Vice Chair Roe moved, seconded by Boardmember Martin, to approve the
request in the amount of $3,00 plus taxes. The motion carried by the
following vote:
Aye: 4 - Chair Cooper, Vice Chair Roe, Boardmember Gies and
Boardmember Martin
Recused: 1 - Boardmember Seward
4.B 15-0142 Long Term Care Rules Revision
The Board asked staff to draft a proposed amendment to the rule on long-term care
for LEOFF I members. Two versions (formal and "plain talk") were presented at the
January meeting. The Board preferred the "plain talk" version. Boardmember Martin
volunteered to review and modify the "plain talk" version. He recommended a small

revision, replacing "needs to" with "must" in the first sentence for final Board approval.
This item was delayed until next month, pending further discussion at the
April 13 meeting under File 15-0365.
4.C 15-0143 Dental Reimbursement Rules Interpretation
Previous LOEFF Boards approved ongoing additional dental cleanings for six
members several years ago. These members have continued to receive quarterly and
semi-annual cleaning reimbursements. Staff asked the Board for dental rule
interpretation to determine if members are required to resubmit the necessary
documentation to seek dental claims reimbursement for additional cleanings approval
from the Board each calendar year. Following discussion, it was determined staff
should ask for medical necessity every two years. Exceptions to the LEOFF rules are
not ongoing.
The decision was approved.
4.D 15-0144 Prescribed Over the Counter Medications
Staff asked the Board for a decision about whether to reimburse members for
over-the-counter medications presecribed by a doctor that are not covered under the
Medicare formulary, such as aspirin, laxatives, cyanocobalamin (B12), etc. The Board
determined we would follow the formulary process to add prescriptions to the
Medicare formulary. If they were not added to the formulary, we could reimburse
members for the cost of over-the-counter prescriptions from a doctor. We must have
proof that the member followed the process to add the prescriptions to the Medicare
formulary.
The decision was approved.
5. REPORTS
5.A 15-0145 LEOFF 1 Expense Report for December 2014
Staff provided the Board with the expense report for December, 2014 for discussion
and review.
The report was received.
6. ADJOURNMENT
