CALL TO ORDER Chair Cooper called the meeting to order at 5:30 p.m. 2. ROLL CALL Present: 5 - Chair Jim Cooper, Vice Chair Jeannine Roe, Boardmember Russ Gies, Boardmember Caleb Martin, and Boardmember Jack Seward 2.A OTHERS PRESENT Carl Watts, Personnel Analyst 3. APPROVAL OF MINUTES 3.A 15-1059 Approval of October 12, 2015 LEOFF Disability Board Meeting Minutes The minutes were unanimously approved. 4. BUSINESS ITEMS 4.A 15-1060 Case #09-15 Long Term Care Vice Chair Roe moved, seconded by Boardmember Seward, to approve case #09-15 long-term care request not to exceed $290/day. The motion was unanimously approved. 4.B 15-1061 Case #10-15 Hearing Aid Request Vice Chair Roe moved, seconded by Boardmember Seward, to approve case #10-15 for hearing aids in the amount of $2,951.94. The motion was unanimously approved. 4.C 15-1062 Case # 11-15 Dental Request Vice Chair Roe moved, seconded by Boardmember Martin, to approve case #11-15 dental request in the amount of $4,945.80, which includes a 90-day fee of $384.80. The motion was unanimously approved. 4.D. 15-1211 Case #12-15 Hearing Aid Request City of Olympia Page 1 LEOFF I Disability Board Meeting Minutes November 9, 2015 Boardmember Martin moved, seconded by Vice Chair Roe, to approve case #12-15 hearing aid request in the amount of $1,799.99 from Costco. The motion was unanimously approved. 5. REPORTS 5.A 15-1063 The September 2015 LEOFF expenses report The Board reviewed and discussed the September 2015 LEOFF expense report.
