Vice Chair Roe called the meeting to order at 5:30 p.m.

Boardmember Jack Seward
Excused: 2 - Chair Jim Cooper, and Boardmember Caleb Martin

2. APPROVAL OF MINUTES
2.A 16-0578 Approval of April 11, 2016 LEOFF Disability Board Meeting Minutes
The minutes were approved.
3. BUSINESS ITEMS
3.A 16-0592 Case #03-16 Dental Request
Boardmember Seward moved, seconded by Boardmember Gies, to approve
Case #03-16 for dental work in the amount of $1,423. The motion passed
unanimously.
4. REPORTS
4.A 16-0593 The LEOFF expense report for March 2016
The report was received.
5. ADJOURNMENT
The meeting adjourned at 5:37 p.m.
