Chair Cooper called the meeting to order at 5:30 p.m.

3. APPROVAL OF MINUTES
3.A 16-1130 Approval of July 11, 2016 LEOFF Disability Board Meeting Minutes
The minutes were approved.
4. BUSINESS ITEMS
4.A 16-1129 Member-at-Large Election
Boardmember Martin agreed to serve the Board for another two-year term.
The Board unanimously re-appointed Caleb Martin to the member-at-large
position for a second term.
5. REPORTS
5.A 16-1132 Report on Hearing Aids Purchased from Costco
Case #06-16 was for a retiree from the police department. Case #07-16 was for a fire
department retiree. Both members obtained two quotes, and the lowest estimates
were from Costco. The prices for the hearing aids were $2,599.98 for each member.
Two LEOFF 1 retirees were approved to purchase hearing aids from Costco
in accordance with the LEOFF board policies and procedures.
