CALL TO ORDER Vice Chair Bateman called the meeting to order at 5:30 p.m. 1.A ROLL CALL Present: 3 - Vice Chair Jessica Bateman, Boardmember Caleb Martin and Boardmember Russ Gies Excused: 1 - Chair Jim Cooper Absent: 1 - Boardmember Jack Seward 2. OTHERS PRESENT Carl Watts, LEOFF Liaison 3. APPROVAL OF MINUTES 17-0640 Approval of the May 8, 2017 LEOFF Disability Board Meeting Minutes The minutes were approved. 6. BUSINESS ITEMS 17-0638 Hearing Aid Procedure Language Change Boardmember Martin moved, seconded by Boardmember Gies, to approve the new hearing aid language that allows LEOFF members to get only the Costco quote if they want hearing aids from Costco. The language calls for the Board to review prices once a year to make sure the prices do not substantially increase. The motion was approved unanimously. 17-0637 Case #09-17 Medical Request Boardmember Gies moved, seconded by Boardmember Martin, to approve the payment /reimbursement for a portable CPAP machine in the amount of $505.62. The motion was approved unanimously. 7.
