Request to APPROVE Change Order No. 1 to the 2019 STORM SEWER
LINING CONTRACT with J.C. DILLON, INC., and to AUTHORIZE the City
Manager to Execute the Change Order to Add to the Budget, in the Amount of

19-374 Communication from the City Manager and Director of Human Resources with
a Request to APPROVE and AUTHORIZE the Execution of a Three-Year
CONTRACT with HUMANA to Administer the EMPLOYEE ASSISTANCE
PROGRAM (EAP).

a Request to APPROVE and AUTHORIZE The City Manager to Enter into a
One-Year CONTRACT with HCC LIFE INSURANCE COMPANY for STOP
LOSS INSURANCE SERVICES for the City’s Health Care Plan to be Effective
January 1, 2020, through December 31, 2020.
19-376 Communication from the City Manager and Director of Community
Development with a Request for the Following:
A. APPROVE a CONTRACT with STANTEC CONSULTING, INC. for the
Implementation of the US Environmental Protection Agency Brownfield
Assessment Grant; and
B. ADOPT an ORDINANCE Amending the City of Peoria 2019 REVISED
ANNUAL BUDGET Relating to the Capital Fund to Recognize the
Receipt of Grant Funding Provided By the US Environmental Protection
Agency Brownfield Assessment Grant and Corresponding Expenditures,
in the Amount of $300,000.00.
19-377 Communication from the City Manager and Director of Public Works with a
Request for the Following:
A. Exercise a Lease Purchase Option with ENTERPRISE FLEET
MANAGEMENT and to APPROVE the PURCHASE of Four Vehicles
Previously Leased from Enterprise Fleet Management, in the Amount of
$95,000.00.
B. ADOPT an ORDINANCE Amending the City of Peoria 2019 REVISED
ANNUAL BUDGET Relating to the Transfer of Funds from the General
Fund to the Capital Fund to Purchase Four (4) Vehicles Previously
Leased from Enterprise Fleet Management, in the Amount of
$95,000.00.
19-378 Communication from the City Manager and Corporation Counsel with a
Request to APPROVE a RESOLUTION Amending Resolution No. 88-240
Pertaining to INDEMNIFICATION.

19-379 Communication from the City Manager and Director of Public Works with a
Request to APPROVE the Following for All Council Districts:
A. A RESOLUTION Amending Resolution No. 18-073-A Setting Fees for
the Peoria Combined and Sanitary Sewer District Increasing the CITY’S
SANITARY SEWER LATERAL FEE from 1.1123/CCF to 1.1902/CCF
Effective May 1, 2020, and from 1.1902/CCF to 1.2735/CCF Effective
May 1, 2021, for Property Owners who are Directly Served by City
Sewers; and
B. A RESOLUTION Amending Resolution No. 18-073-B Setting Fees for
the Peoria Sanitary Sewer Rehabilitation District Increasing the CITY
SEWER REHABILITATION RATE from 1.1931/CCF to 1.2766/CCF
Effective May 1, 2020, and from 1.2766/CCF to 1.3660/CCF Effective
May 1, 2021, for All City Property Owners who are Directly Served by
Greater Peoria Sanitary District (GPSD) or City Sewers.
19-380 Communication from the City Manager and Director of Public Works with a
Request to APPROVE a STATE MOTOR FUEL TAX RESOLUTION, in the
Amount of $2,150,000.00, for Construction Services for the 2020 SHERIDAN
ROAD PROJECT (Eleanor Place to Richmond Avenue). (Council District 2)
19-381 Communication from the City Manager and Director of Public Works with a
Request to APPROVE a STATE MOTOR FUEL TAX RESOLUTION, in the
Amount of $557,500.00, for Construction and Construction Engineering
Services for the ALLEN ROAD RESURFACING PROJECT (Walton Way to 800
feet south of Altorfer Drive). (Council District 5)
19-382 Communication from the City Manager and Director of Community
Development with a Request to Concur with the Recommendation from the
Planning and Zoning Commission and Staff to ADOPT an ORDINANCE
Amending an Existing SPECIAL USE Ordinance No. 14,289, as Amended, in a
Class R4 (Single Family Residential) District and a Class R2 (Single Family
Residential) District to Add a Wall Sign and Light Standard Signs for the
