Smith, Previously Married to Firefighter Matthew D. Smith, with a Request to APPROVE. ITEM NO. 4 REQUEST to APPROVE the REMOVAL from the PENSION ROLLS of THOMAS C. JACKSON, JR. Due to His Death on JUNE 25, 2021, and to Make Payments to the Thomas C. Jackson, Jr. Estate in the Amount of $5,173.50 for 25 Days of June 2021. Recommendation: To RECEIVE and FILE the Death Certificate and to APPROVE the Payment of $5,173.50 for 25 days of June. ITEM NO. 5 REQUEST to ACCEPT the APPLICATION for a WIDOW’S PENSION BENEFIT for BONNIE L. JACKSON, She being the Widow of THOMAS C. JACKSON, JR., with Benefits Commencing June 26, 2021, with her Benefit Based on 100% of his monthly benefit of $6,208.15 per month; therefore Benefits for 5 days of June to be Paid, in the amount of $1,034.70 and $6,208.15 for each month thereafter. Recommendation: APPROVE the Payment $1,034.70 for 5 Days of June and $6,208.15 for each month thereafter. ITEM NO. 9 CONTINUED DISCUSSION Regarding the CERTIFIED TRUSTEE PROGRAM for New Trustees Provided Virtually, offered by the NIU Outreach. RECOMMENDATION: To Allow New Trustees to Attend.
