LICENSING BOARD MEETING MINUTES – DRAFT
A meeting of the Licensing Board was held on May 22, 2023 at 6:00 p.m. In attendance were
Chairman Gary Barrett and members Deborah Greel and Gary Santo, Melissa Mojica, Clerk of the
Board and Special Investigator, Sgt. Mike Ball.
1. Application for a One Day Liquor License at OMEN, 184 Essex St. on June 24, 2023 from 6-7pm
for 30 people for a Sabbat Celebration.
Megan Lecuru, the manager presented the application. She said she is TIPS certified and will be
serving and will be sending the Certificate of Insurance. Mr. Santo motioned to approve pending
Certificate of Insurance. Ms. Greel seconded. All in favor. Roll call vote:3-0.
2. Application for a One Day Liquor License at House of the Seven Gables, 115 Derby St. on June
24, 2023 from 5-10pm for 50 people for a Wedding Reception.
Emily Simpson presented the application. She said this is for her wedding reception. She said
Perfect Pour will be serving and she has submitted the Certificate of Insurance and has arranged
for a police detail. Ms. Greel motioned to approve. Mr. Santo seconded. All in favor. Roll call
vote: 3-0.
3. Application for a One Day liquor License at Old Town Hall on June 8, 2023 from 5-8pm for 70
people for a reception. Applicant: Nicole McClain, Northshore Juneteenth Association.
Nicole McClain presented the application. She said the event will be held at the Visitor Center on
New Liberty St. for about 50 people. Mr. Barrett asked who would be serving. Ms. McClain said
that Neil Whitridge is TIPS certified and will be serving. Mr. Santo motioned to approve and
waive the fee. Ms. Greel seconded. All in favor. Roll call vote: 3-0
4. Application for a One Day Liquor License at 10 White St. on June 9, 2023 from 5-8pm for 100
people for an employee party. Applicant: Safe Harbor Hawthorne Marina.
Erik Auerbach presented the application. He said he is the owner of Head Tap Beer Truck and he
will be pouring on site for the event and he has two certified bartenders. Ms. Mojica said they still
need to provide the Insurance Certificate. Ms. Greel motioned to approve pending insurance. Mr.
Santo seconded. All in favor. Roll call vote: 3-0.
5. Application for a One Day Liquor License at the Forest River Park Function Room on June 11,
2023 from 2-6pm for 40-50 people for a Private Party. Applicant: Sarah Gaddipati.
City Solicitor Rennard/Licensing Board Agent requested Ms. Mojica present the application as
listed above on behalf of the Park and Recreation Function Manager and noted that the applicant
will contact the Police Department for a detail and provide the City with a Certificate of Liquor
Liability. Ms. Greel motioned to approve pending insurance and detail. Mr. Santo seconded. All in
favor. Roll call vote: 3-0.
6. Application for a One Day Liquor License at Forest River Park Function Room on June10, 2023
from 12-6pm for 40-50 people for a Private Party. Applicant: William Mallette.
As above, Ms. Rennard requested Ms. Mojica to present the application as listed above and noted
that the applicant will contact the Police Department for a police detail and provide the City with a
Certificate of Liquor Liability. Mr. Santo motioned to approve pending insurance and detail. Ms.
Greel seconded. All in favor. Roll call vote: 3-0.
7. Application for multiple One Day Liquor License at House of the Seven Gables, 115 Derby St.
Applicant: Deb Costa
*June 3 – Benefit Event/Wine Tasting – 100 people – 5-8pm
*July 4 – Settlement Event – 100 people – 5-10pm
*August 13 – Settlement Event – 100 people - 5-10pm
*September 19 – Settlement Event – 100 people - 5-10pm
Deb Costa presented the applications. Ms. Mojica said that all of the paperwork was in order. Mr.
Santo motioned to approve. Ms. Greel seconded. All in favor. Roll call vote 3-0.
8. Application for a sub licensee fortune teller at Ascend, 192 Essex St. Applicant: Elizabeth
Simmons.
Elizabeth Simmons presented the application. She said she is looking to do readings at ASCEND
on Essex St. Ms. Mojica said all of the paperwork was in order. Mr. Santo motioned to approve.
Ms. Greel seconded. All in favor. Roll call vote: 3-0.
9. Application for a sub licensee fortune teller at Hermetic Arts, 272 Essex St. Applicant: Katherine
Ciccolini.
Ms. Ciccolini presented the application. She said she is looking to do readings at Hermetic Arts on
Essex St. Ms. Mojica said she is waiting for the CORI. Ms. Greel motioned to approve pending
CORI. Mr. Santo seconded. All in favor. Roll call vote: 3-0.
10. Application for an Individual fortune teller license at 86B Wharf St. Applicant: Nicole Fronte.
Ms. Fronte presented the application and said she will be reading from her office space at 8 Front
St, unit #216 so wanted the application to reflect that change. Ms. Mojica said she is waiting for
the CORI. Mr. Santo motioned to approve pending CORI. Ms. Greel seconded. All in favor. Roll
call vote: 3-0.
11. Application for an Individual fortune teller license at 192 Lafayette St. Apt.#2. Applicant: Patricia
Rainville.
Ms. Rainville said she is a life coach and sometimes reads tarot cards at her home. Ms. Mojica
said all of the paperwork is in order. Ms. Greel motioned to approve. Mr. Santo seconded. All in
favor. Roll call vote: 3-0.
12. Application for an Entertainment License at 76 Lafayette St. Applicant: Couch Dog Brewing.
Adam Shoemaker presented the application. He said they are looking for piped in music through
speakers and a movie screen projector for one wall. Ms. Mojica asked where the speakers are
placed because there are residential units above. Mr. Shoemaker said they are high on the ceiling.
Mr. Barrett said sound flows. Ms. Greel asked about the hours. Mr. Shoemaker said the hours they
are open, which would be 7 days a week from noon-10pm. Ms. Greel asked why they are closing
at 10. Mr. Shoemaker said they are a brewery not a bar. He said they would also like to have a
shuffleboard that will be free of charge to play. Mr. Santo motioned to approve. Ms. Greel
seconded. All in favor. Roll call vote: 3-0.
13. Approval of Meeting minutes from April 24 and May 8, 2023.
Ms. Greel motioned to approve April 24th minutes and requested the May 8th meeting be held for
approval until the next meeting. Mr. Santo seconded. All in favor. Roll call vote: 3-0
14. Old/New Business/Adjournment:
Sgt. Ball said there have been some complaints regarding noise at East Regiment Beer Company
on Church St. and him and Ms. Mojica will be going out to check on them.
Adjournment: Ms. Greel motioned to approve. Mr. Santo seconded. All in favor. Roll call vote: 3-
0.

Notice is hereby given that a meeting of the Salem Licensing Board will be held on Monday, May
22, 2023 at 6:00 p.m. via remote participation in accordance with Chapter 2 of the Act of 2023. Every
