CONSENT ITEMS 12.1 MOTION - CONTRACT AWARD - PAVEMENT PREVENTIVE MAINTENANCE 2020 RECOMMENDATION: It is recommended by the Transportation and City of Santa Rosa Regular Meeting Agenda and Summary Report Page 4 of 6 City Council - CANCELLED MARCH 17, 2020 Public Works Department that the Council, by motion, award Contract No. C02252, Pavement Preventive Maintenance 2020, in the amount of $736,007.00 to the lowest responsible bidder, Sierra Nevada Construction, Inc. of Sparks, Nevada, approve a 15% contract contingency, and authorize a total contract amount of $846,408.05. 12.2 RESOLUTION - PROFESSIONAL SERVICES AGREEMENT WITH CARLILE MACY, INC. TO PROVIDE LANDSCAPE ARCHITECTURAL DESIGN SERVICES TO PREPARE CONSTRUCTION DOCUMENTS FOR SIX FIRE DAMAGED PARK/LANDSCAPE SITES RECOMMENDATION: It is recommended by the Transportation and Public Works Department, Parks Division, that Council, by resolution, approve the Professional Services Agreement between the City of Santa Rosa and Carlile Macy, Inc. of Santa Rosa, California in an amount not to exceed $314,060 to provide landscape architectural design services to prepare construction documents for six park/landscape sites which were damaged by the October 2017 wildfires. 12.3 RESOLUTION - APPROVAL OF PURCHASE ORDER FOR FIFTEEN 12-LEAD EKG MONITOR/DEFIBRILLATOR UNITS AND ACCESSORIES RECOMMENDATION: It is recommended by the Fire and Finance Department that the Council, by resolution, approve a Purchase Order with Stryker Sales Corporation of Portage, Michigan, for fifteen 12-Lead EKG Monitors/Defibrillator Units and Accessories, in an amount not to exceed $529,750.46. 12.4 RESOLUTION - REQUEST AUTHORIZATION TO SUBMIT A 2019 YOUTH REINVESTMENT GRANT PROGRAM APPLICATION TO THE BOARD OF STATE AND COMMUNITY CORRECTIONS RECOMMENDATION: It is recommended by the Community Development and Engagement Portfolio, that the Council, by resolution, 1) authorize the submission of the 2019 Youth Reinvestment Grant application to the Board of State and Community Corrections in the amount of $2,000,000 for the award period of July 1, 2020 - June 30, City of Santa Rosa Regular Meeting Agenda and Summary Report Page 5 of 6 City Council - CANCELLED MARCH 17, 2020 2023, 2) approve a Grant Agreement with the Board of State and Community Corrections in a form approved by the City Attorney, 3) commit to satisfy matching fund requirements, and 4) delegate authority to the Assistant City Manager or their designee to submit the application and execute the Grant Agreement and any amendments that are in a form approved by the City Attorney. 12.5 ORDINANCE ADOPTION SECOND READING- ORDINANCE OF THE COUNCIL OF THE CITY OF SANTA ROSA AMENDING TITLE 20 OF THE SANTA ROSA CITY CODE BY RECLASSIFYING THE PROPERTIES FOR THE CARITAS VILLAGE PROJECT, LOCATED AT 431, 437, 439, 465 A STREET, AND 506, 512, 516, 520, 600, 608, AND 612 MORGAN STREET TO THE TV-M-H-SA (TRANSIT VILLAGE-MIXED USE) ZONING DISTRICT, FILE NO. PRJ18-052 RECOMMENDATION: This ordinance, introduced at the March 3, 2020, Regular Meeting by a (5-0-0-2) vote (Council Members Olivares and Sawyer abstained/recused), amends Title 20 of the Santa Rosa City Code by reclassifying the properties for the Caritas Village Project, located at 431, 437, 439, 465 A Street, and 506, 512, 516, 520, 600, 608, and 612 Morgan Street to the TV-M-H-SA (Transit Village-mixed use) Zoning District, File No. PRJ18-052. 13.
