Appearance by Sonia Verma. Discussion ensued regarding Ms. Verma's experience
with operating a convenience store. She was reminded to check identification for all
alcohol and cigarette sales.

Appearance by Christopher D. Surges. Discussion ensued regarding Mr. Surges'
experience, the type of music and food they will have, and the remodeling taking place.

report and the amount of time since his last incident. Mr. Fearen was reminded that no
alcohol is allowed outside the establishment, the doors should remain closed and to
clean up around the area. He was told the business needs to be respectful of the
neighbors.

Check involving thirteen (13) West Allis businesses, resulting with three (3)
selling tobacco to an underage person without requesting identification
(2014-2015 Cigarette License no. 1014 of National Quick Food Mart, LLC,
Bhola Singh, Agent, d/b/a Kwik Pantry, premises located at 5631 W. National
Ave.).
Appearance by Bhola Singh. Mr. Singh informed the Committee that the employee has
been fired for selling cigarettes to an underage person.
This matter was Recommended to be Placed on File on a Block Vote.
66. 2015-0247 Police Department Report regarding March 22, 2015 Tobacco Compliance
Check involving thirteen (13) West Allis businesses, resulting with three (3)
selling tobacco to an underage person without requesting identification
(2014-2015 Cigarette License no. 2110 of In and Out Grocery LLC, Amjad A.
Jamal, Agent, d/b/a In and Out Grocery, premises located at 5932 W. Burnham
St.).
Appearance by Amjad A. Jamal. Mr. Jamal informed the Committee that his brother
was the employee that sold cigarettes to an underage person.
This matter was Recommended to be Placed on File on a Block Vote.
67. 2015-0248 Police Department Report regarding March 22, 2015 Tobacco Compliance
Check involving thirteen (13) West Allis businesses, resulting with three (3)
selling tobacco to an underage person without requesting identification
(2014-2015 Cigarette License no. 1078 of Shivkrupa, LLC, Pravinkumar R.
Patel, Agent, d/b/a Express Pantry, premises located at 8530 W. Greenfield
Ave.).
Appearance by Pravinkumar R. Patel. Mr. Patel informed the Committee that the
employee has been fired for selling cigarettes to an underage person.
This matter was Recommended to be Placed on File on a Block Vote.

the preceding items on a Block Vote. The motion carried by the following vote:

Nonappearance by Mary Louise Eli. Ms. Eli did not contact the Clerk's Office. The
Committee requested she be ordered to appear at the next License & Health meeting.
Nonappearance of Charline Hart. Ms. Hart notified the Clerk's Office that she was
working on obtaining her WI State Seller's Permit.

the Clerk's Office stating she would pay the delinquent personal property taxes on
April 22, 2015.
Second nonappearance of Krap Shoot, Diane Plumeri. Ms. Plumeri contacted the
Clerk's Office and said she would pay the delinquent personal property taxes.
Second nonappearance of Sacred Skin, Marcello Orlando. Mr. Orlando contacted the
Clerk's Office and said he would be in April 22, 2015 to pay the delinquent personal
property taxes.
