Branson, Agent, Incident Date: April 18, 2015. Appearance by Lisa M. Branson. Discussion ensued regarding the police report and the patron being described in the report as extremely intoxicated. Ms. Branson was reminded to have her employees monitor the outside area for trash and anything else that may need to be cleaned up. A motion was made by Ald. Roadt, seconded by Ald. Salem, Agent, d/b/a 9801 LLC, for premises located at 9801 W. Dakota St. (new-nonexisting location). Nonappearance of Hasan S. Salem. Mr. Salem contacted the Clerk's Office. The Committee requested he be ordered to appear at the next scheduled License & Health meeting. This matter was Held. 9. 2015-0331 2015-2017 Operator’s License (bartender/sales clerk) renewal application no. 17-004 for Gregory S. Dowden. Appearance by Gregory S. Dowden. Discussion ensued regarding Mr. Dowden's police report. He was told that if he receives a third OWI his Operator's (bartender/sales clerk) License may be revoked. Mr. Dowden was told he needs to make good decisions for himself and his patrons. A motion was made by Ald. Barczak, seconded by Ald. Roadt, that this matter was Recommended For Approval. Weigel No: 0 10. 2015-0332 2015-2017 Operator’s License (bartender/sales clerk) renewal application no. 17-042 for Colleen C. Doolan. Appearance by Colleen C. Doolan. Discussion ensued regarding Ms. Doolan's police report. A motion was made by Ald. Haass, seconded by Ald. Barczak, that this matter was Recommended to be Referred for Summons and Complaint to the City Attorney's Office for Revocation. The motion carried by the following vote: Aye: 3- Ald. Barczak, Ald. Haass, and Ald. Weigel No: 2- Ald. Czaplewski, and Ald. Roadt 11. 2015-0381 2015-2017 Operator’s License (bartender/sales clerk) renewal application no. 17-158 for Gary D. Johnson. Appearance by Gary D. Johnson. Discussion ensued regarding Mr. Johnson's police report. Mr. Johnson stated he serves for Daniel's Catering and doesn't tend bar on a regular basis. The Committee asked to hold this application until the outcome of the pending charges. Mr. Johnson will be asked to appear at every License & Health regular scheduled meeting for an update from Mr. Johnson until the charges are finalized. This matter was Held. Discussion ensued regarding Ms. Swaney's police report and the lengthy list of violations while operating a motor vehicle. She was told to obey the laws and to make smart decisions. Discussion ensued regarding Ms. Mark's police report and her violations. Ms. Mark's was told her application was not filled out completely because she did not list any of her violations. She was reminded to fill out any applications truthfully and completely and to be forthcoming with her answers. A motion was made by Ald. Barczak, seconded by Ald. Weigel No: 0 19. 2015-0289 2015-2016 Operator’s License (bartender/sales clerk) application no. 16-300 for Ramanlal Patel. Appearance by Ramanlal Patel. Discussion ensued regarding the police report and the sixty (60) day WI Driver's License requirement. Mr. Patel was asked to provide a lease or utility bill, and a WI Driver's License to the Clerk's Office before his bartender/sales clerk license application would be approved. This matter was Held. Appearance by Ranjit Singh. Discussion ensued regarding Mr. Singh's Driver's License. Mr. Singh was told that the International Driver's License is not valid unless accompanied by a state driver's license. Mr. Singh told the Committee the DMV would not issue him a WI Driver's License because he did not have a Social Security number. Mr. Singh was told to get a social security card, WI Driver's License and provide the Clerk's Office with proof before his bartender/sales clerk license application would be approved. Mr. Singh was told he could not drive until he received a valid WI Driver's License. This matter was Held. 20. 2015-0290 2015-2016 Operator’s License (bartender/sales clerk) application no. 16-292 for Rey A. Khan. Appearance by Rey A. Khan. Discussion ensued regarding Mr. Khan's police report. A motion was made by Ald. Weigel, seconded by Ald. Haass, that this matter was Recommended For Approval. Ms. Jacobson did not contact the Clerk's Office. The Committee requested she be ordered to appear at the next scheduled License & Health meeting. This matter was Held. F. MATTERS FOR DISCUSSION/ACTION 24. 2015-0385 Discussion relative to non-payment of real estate taxes for Teamtam, LLC/Dopp’s, Tammy Dopp, dated May 21, 2015. Nonappearance of Tammy Dopp. Ms. Dopp contacted the Clerk's Office. The Committee requested she be ordered to appear at the next scheduled License & Health meeting. Appearance by Pamela Uebelacker. Ms. Uebelacker informed the Committee the business has been closed since April 30, 2015 and they have been foreclosed. She said they are in the process of moving out, filing for bankruptcy and would not be renewing the license. This matter was Discussed and Consensus Given. D. CLOSED SESSION A closed session was not held. 2. 2015-0384 Stipulation In the Matter of the Complaint Against Carolyn Wardius, d/b/a New Life Cat Rescue. Attachments: Stipulation New Life Cat Rescue This matter was Recommended for Approval on a Block Vote. 3. 2015-0116 Summons and Complaint in the matter of the complaint against Carolyn Wardius, for the premises located at 7911 W. Becher St., d/b/a New Life Cat Rescue (Animal-Related Permit).
