Haass, that this matter be Approved. The motion carried unanimously. D. NEW AND PREVIOUS MATTERS 2. 2016-0516 2016-2017 Direct Sellers/Solicitors Permit application no. 17 of Anthony Vazquez, Jr. Second nonappearance of Anthony Vazquez, Jr. Mr. Vazquez did not contact the Clerk's Office. Recommended denial based on the police report. A motion was made by Ald. Weigel, seconded by Ald. Roadt, that this matter was Recommended for Denial. Haugen, d/b/a Resale & Oddities, for premises located at 7415 W. Becher St. Appearance by Eric L. Haugen. Discussion ensued regarding Mr. Haugen's police report, what sources Mr. Haugen uses to acquire merchandise for his store, maintaining records either handwritten or electronic and NEWPRS (Northeastern Wisconsin Property Reporting System). Mr. Haugen was reminded to follow City Ordinances and State Laws regarding how he acquires merchandise and reporting acquired merchandise. A motion was made by Ald. Haass, seconded by Ald. Roadt, that this matter was Recommended for Approval subject to the necessary requirement(s). The motion carried by the following vote: Aye: 4- Ald. Czaplewski, Ald. Roadt, Ald. Haass, and Ald. Weigel No: 0 4. 2016-0520 Police Department Report involving Brass Monkey, 11904 W. Greenfield Ave., of ZJ Squared Ventures LLC, John G. Mackowski, Agent, Incident Date: July 9, 2016. Appearance by John G. Mackowski. Discussion ensued regarding the police report. Mr. Mackowski was reminded it is illegal to serve alcohol to an intoxicated person. A motion was made by Ald. Weigel, seconded by Ald. Roadt, that this matter was Recommended to be Placed on File. The motion carried by the following vote: Aye: 4- Ald. Czaplewski, Ald. Roadt, Ald. Haass, and Ald. Weigel No: 0 5. 2016-0523 Police Department Report involving Town Beer & Liquor, 907 S. 84 St., of BHP Partners LLC, Hitesh V. Patel, Agent, Incident Date: July 4, 2016 (Class B Tavern License). Nonappearance of Hitesh V. Patel. Mr. Hitesh did not contact the Clerk's Office. The Committee requested he be ordered to appear at the next scheduled License & Health meeting. This matter was Held 6. 2016-0564 2016-2017 Operator’s License (bartender/sales clerk) application no. 17-501 of Nicole M. Hoffmeier. Appearance by Nicole M. Hoffmeier. Discussion ensued regarding Ms. Hoffmeier's police report and the items she failed to list on her application regarding arrests, convictions or fines. Ms. Hoffmeier was reminded to call the police when there is trouble, to be cooperative with the police and to check identification. A motion was made by Ald. Haass, seconded by Ald. Shay. Nonappearance of Stephanie A. Shay. Ms. Shay did not contact the Clerk's Office. The Committee requested she be ordered to appear at the next scheduled License & Health meeting. This matter was Held 8. 2016-0494 2016-2018 Operator’s License (bartender/sales clerk) application no. 18-194 of Abigail R. Sindelar. Second nonappearance of Abigail R. Sindelar. Ms. Sindelar did not contact the Clerk's Office. Recommended denial based on two (2) nonappearances. A motion was made by Ald. Weigel, seconded by Ald. Roadt, that this matter was Recommended for Denial. The motion carried by the following vote: Aye: 4- Ald. Czaplewski, Ald. Roadt, Ald. Haass, and Ald. Weigel No: 0 9. 2015-0381 2015-2017 Operator’s License (bartender/sales clerk) renewal application no. 17-158 for Gary D. Johnson. Nonappearance by Gary D. Johnson. Mr. Johnson informed the Clerk's Office of the decision from his trial. Mr. Johnson was asked to submit a letter to withdraw his application. A motion was made by Ald. Weigel, seconded by Ald. Roadt, that this matter was Recommended for Denial based on the police report. The motion carried by the following vote: Aye: 4- Ald. Czaplewski, Ald. Roadt, Ald. Haass, and Ald. Weigel No: 0 E.
