what steps Ms. Lopez might do in the future to prevent this type of incident.

Incident Date: January 1, 2017.
Nonappearance of Tonja M. Bassett. Ms. Bassett contacted Ald. Czaplewski. The
Committee requested she be ordered to appear at the next scheduled License & Health
meeting.

4. 2016-0717 Police Department Reporting involving The Hideout, 6201 W. Mitchell St., of
Hideout West Allis, Inc., Daniel R. Seiy, Agent, Incident Date: October 30,
2016.
Appearance by Daniel R. Seiy. Discussion ensued regarding the police report.

Discussion ensued regarding the police report. Lt. Saftig advised that the police
should have been called.

Appearance by Megan S. Brown. Discussion ensued regarding the police report and
Ms. Brown's allegations against Remedy Bar.
A motion was made by Ald. Roadt, seconded by Ald. Haass, that this matter was
Recommended to be Placed on File. The motion carried by the following vote:

8. 2017-0055 2016-2017 Class B Tavern License application no. 2398 of Top Dog
Enterprises, LLC, Ronald L. Mellantine, Agent, d/b/a The Choice Eatery and
Entertainment, for the premises located at 9638 W. National Ave. (new-existing
location).
Attachments: Top Dog Enterprises Affidavit of Publication
Appearance by Ronald L. Mellantine. Discussion ensued regarding Mr. Mellantine's
progress and anticipated opening date. Attorney Cerwin commented on whether Mr.
Mellantine had to wait twelve (12) months since revocation before reapplying.
The Committee decided to hold Mr. Mellantine's application until the next License &
Health meeting while this matter is investigated.

9. 2016-0451 2016-2017 Entertainment Club License application no. 2346 of Just For Today
Recovery Club, Inc., Fred D. Bell, Agent, d/b/a Just For Today Recovery Club,
for the premises located at 1900 S. 60 St.
Nonappearance of Fred D. Bell. Discussion ensued regarding the police report of Mr.
Bell's and the conviction.
A motion was made by Ald. Weigel, seconded by Ald. Haass, that this matter was Referred
for Summons and Complaint to the City Attorney. The motion carried by the following
vote:

Poniewaz, Agent, d/b/a Shenanigans, for the premises located at 5630 W.
Lincoln Ave. (new-existing location).

Second nonappearance of George Poniewaz. Mr. Poniewaz called the Clerk's Office
and said he was not pursuing this license. Mr. Poniewaz was advised to write a letter
withdrawing his application.

14. 2017-0108 Discussion regarding 2016-2017 Class B Tavern Licenses not yet issued
(2-28-17).
2016-2017 Class B Tavern License applications:
Second nonappearance of Susan J. Fazen, Agent, 1641 S. 68 St., d/b/a The Dog House,
Council approval date August 2, 2016. Ms. Fazen did not contact the Clerk's Office.
A motion was made by Ald. Roadt, seconded by Ald. Haass to refer this matter for
Summons and Complaint to the City Attorney's Office.
The motion carried by the following vote:
Aye: 4 - Ald. Czaplewski, Ald. Roadt, Ald. Haass, and Ald. Weigel
No: 0
Appearance by David P. Bickler, Agent & Theodore M. Galaszewski, 5630 W. Lincoln
Ave., d/b/a Bobblehedz, Council approval date May 17, 2016. Discussion ensued
regarding the requirements needed to reopen Bobblehedz. Mr. Bickler and Mr.
Galaszewski explained to the Committee their plans to begin the process to reopen the
business, which includes applying for a Special Use Permit, and that they have the
business and building for sale with an agent. Mr. Bickler and Mr. Galaszewski told the
Committee the prior applicant withdrew his offer to purchase the business.
Nonappearance of Gerald M. Pfeifer, Agent, 1101 S. 60 St., d/b/a Hawley House,
Council approval date November 15, 2016. Mr. Pfeifer contacted the Clerk's Office
and Ald. Czaplewski.
Appearance by Nabil R. Alfahel, Agent, 3411 S. 108 St., d/b/a Classic Reality of West
Allis, Council approval date September 20, 2017. Discussion ensued regarding
Mr.Alfahel's progress with remodeling the building. Mr. Alfahel informed the
Committee he is waiting for his outside building permit to be approved and addressed
unauthorized cars parking in the lot with for sale signs on them.
