This matter was Recommended to be Placed on File on a Block Vote. Passed The Block Vote A motion was made by Ald. Roadt, seconded by Ald. Weigel, to approve all the actions on the preceding items on a Block Vote. The motion carried by the following vote: Aye: 5- Ald. Czaplewski, Ald. Roadt, Ald. Haass, Ald. May, and Ald. Weigel No: 0 41. 2017-0792 2018 Gun Range License renewal application no. 29 of The Shooters Shop, Inc., 2465 S. 84 St., Kevin M. Nugent, Agent. A motion was made by Ald. May, seconded by Ald. Weigel, that this matter was Recommended for Approval subject to the necessary requirement(s). The motion carried by the following vote: Aye: 5- Ald. Czaplewski, Ald. Roadt, Ald. Haass, Ald. May, and Ald. Weigel No: 0 42. 2017-0782 License Applications. 2017-2018 and 2017-2019 Operator’s License (bartender/sales clerk) applications: Lane, Christine M.; Mather, Lisa Ann; Rocque, Matthew D.; and White, Gail E. A motion was made by Ald. Haass, seconded by Ald. May, that this matter was Recommended for Approval subject to the necessary requirement(s). The motion carried by the following vote: Aye: 5- Ald. Czaplewski, Ald. Roadt, Ald. Haass, Ald. May, and Ald. Weigel No: 0 43. 2017-0773 2017-2018 Combination Class A Liquor License application no. 2518 of Tall Guy and a Grill, LLC, Daniel P. Nowak, Agent, d/b/a Tall Guy and a Grill, for premises located at 6735 W. Lincoln Ave. (new-nonexisting location). Attachments: AFFIDAVIT OF PUBLICATIONS - Tall Guy and A Grill Appearance by Daniel P. Nowak. A motion was made by Ald. May, seconded by Ald. Beloit Rd., of The 5 Spot, LLC, Christina I. Lopez, Agent, Incident Date: October 3, 2017. Second nonappearance of Christina I. Lopez. Ms. Lopez contacted the Clerk's Office. The Committee requested she be ordered to appear at the next scheduled License & Health meeting. This matter was Held. 45. 2017-0723 2017-2019 Operator’s License (bartender/sales clerk) application no. 19-359 of Sarah L. Groh. Appearance by Sarah L. Groh. A motion was made by Ald. Haass, seconded by Ald. May, that this matter was Recommended for Approval. The motion carried by the following vote: Aye: 5- Ald. Czaplewski, Ald. Roadt, Ald. Haass, Ald. May, and Ald. Weigel No: 0 46. 2017-0733 2017-2018 Operator’s License (bartender/sales clerk) application no. 18-302 of Nathan M. Zetting. Second nonappearance of Nathan M. Zetting. Mr. Zetting did not contact the Clerk's Office. A motion was made by Ald. Roadt, seconded by Ald. Weigel, that this matter was Recommended for Denial based on the record check and two (2) nonappearances. The motion carried by the following vote: Aye: 5- Ald. Czaplewski, Ald. Roadt, Ald. Haass, Ald. May, and Ald. Weigel No: 0 47. 2017-0748 2017-2018 Operator’s License (bartender/sales clerk) application no. 18-313 of Orlin J. Root-Thalman, III. Second nonappearance of Orlin J. Root-Thalman, III. Mr. Root-Thalman did not contact the Clerk's Office. A motion was made by Ald. May, seconded by Ald.
