A. CALL TO ORDER
The meeting was called to order by Chair Czaplewski at 6:00 p.m.

C. APPROVAL OF MINUTES
1. 2018-0211 Minutes (draft) of the License & Health regular meeting of January 23, 2018, recess
meetings of January 16, 2018, February 6, 2018, February 20, 2018, Mach 6, 2018
and March 20, 2018.

A motion was made by Ald. Weigel, seconded by Ald. Haass, that this matter be Approved. The
motion carried unanimously.
All in attendance were told of the importance of calling 911 for any signs of
trouble, encouraged to call and to be helpful and cooperative with the police
when they arrive. They were informed of the City's Compliance Check Programs.

D. NEW AND PREVIOUS MATTERS
2. 2018-0059 2017-2018 Operator’s License (bartender/sales clerk) application no. 18-327 of
Jennifer Lynn Schroeder.
Appearance by Jennifer Lynn Schroeder. Discussion ensued regarding Ms.
Schroeder's background report. Ms. Schroeder was reminded to make good
choices for her customers and to call the police for any problems in the
establishment.
A motion was made by Ald. Weigel, seconded by Ald. Haass, that this matter was Recommended
For Approval. The motion carried by the following vote:

3. 2018-0261 2017-2019 Operator’s License (bartender/sales clerk) application no. 19-419 of
Rosa Maria Gomez Carranza.
Nonappearance of Rosa Maria Gomez Carranza. Ms. Gomez Carranza did not
contact the Clerk's Office. The Committee requested she be ordered to appear at
the next scheduled License & Health meeting.
This matter was Held
4. 2018-0259 2017-2019 Operator’s License (bartender/sales clerk) application no. 19-426 of
Melissa L. Belsha.
Nonappearance of Mellisa L. Belsha. Ms. Belsha did not contact the Clerk's
Office. The Committee requested she be ordered to appear at the next scheduled
License & Health meeting.

5. 2018-0260 2017-2018 Salvage & Recycling Center License application no. 2541 of Electric
Avenue Automotive, Jonathon R. Liebsch, Agent, d/b/a Electric Ave. Auto, premises
located at 5237 W. Electric Ave.
Appearance by Jonathon R. Liebsch. Discussion ensued regarding Mr. Liebsch's
use for the Salvage & Recycling Center License. Mr. Liebsch stated he needed
this license to get his state license to tow scrap cars for buying and selling. He
stated he would be selling the whole car as scrap and not parts and pieces.
A motion was made by Ald. Haass, seconded by Ald. Weigel, that this matter was Recommended
for Approval subject to the necessary requirement(s). The motion carried by the following vote:

6. 2018-0262 Police Department Report regarding the month of February 2018 Tavern Compliance
Checks involving forty-four (44) West Allis businesses, resulting with two (2) selling
alcohol to an underage person without requesting identification (JC’S Pub, Inc.,
Jeffery M. Clark, Agent, d/b/a JC’s Pub, premises located at 8028 W. National
Ave.). Incident date: February 28, 2018.
Appearance by Jeffery M. Clark. Discussion ensued regarding the police report.
A motion was made by Ald. Haass, seconded by Ald. May, that this matter was Recommended to
be Placed on File. The motion carried by the following vote:
Aye: 5- Ald. Czaplewski, Ald. Roadt, Ald. Haass, Ald. May, and Ald. Weigel

7. 2018-0206 Police Department Report regarding the month of February 2018 Tavern Compliance
Checks involving forty-four (44) West Allis businesses, resulting with two (2) selling
alcohol to an underage person without requesting identification (Shepherd’s Sports
Lounge, Inc., Christina M. Knueppel, Agent, d/b/a Shepherd’s Hideaway, premises
located at 8924 W. Schlinger Ave). Incident Date: February 10, 2018.
Appearance by Christina M. Knueppel. Discussion ensued regarding the police
report, the circumstances that left the premise without a licensed bartender on
duty and the employees listed in the police report.
A motion was made by Ald. Weigel, seconded by Ald. Haass, that this matter was Recommended
to be Placed on File. The motion carried by the following vote:

8. 2018-0204 Police Department Report involving Z’s Bar, 6309 W. National Avenue, Z’s Bar,
LLC, David Zarate, Agent, Incident Date: February 11, 2018.
Appearance by David Zarate. Discussion ensued regarding the police report, the
licensed bartender on duty and the new security camera system that Z's Bar had
installed.
A motion was made by Ald. Weigel, seconded by Ald. Haass, that this matter was Recommended
to be Placed on File. The motion carried by the following vote:

E. MATTERS FOR DISCUSSION/ACTION
9. 2018-0263 Discussion regarding noise complaints in the area of 6922 W. Orchard St., relative to
the Class B Tavern licensed establishment, Toms Foolery, Thomas J. Lubas, Agent.
Appearance by Thomas J. Lubas. Discussion ensued regarding the numerous
noise complaints being received and suggestions for alleviating the problem. Mr.
Lubas was asked to contact the apartment manager for more details on the
complaints and work out a solution.
A motion was made by Ald. Haass, seconded by Ald. Weigel, that this matter was Placed on File.
The motion carried by the following vote:

10. 2018-0264 Discussion relative to a memorandum from the Health Department Environmentalist
Coordinator for issues occurring at a Class B Tavern Licensed establishment, Sarge’s
Corner, Ricky E. Sergeant, Agent, premises located at 1479 S. 54 St.
Attachments: Sarge's Corner Memo 3-9-18
Appearance by Ricky E. Sergeant. Laura Temke, West Allis Health Department,
informed the Committee of the repeated and numerous violations in which it
appeared no attempt was made to correct at the establishment. Discussion ensued
regarding the pictures Ms. Temke submitted of the violations and whether Mr.
Sergeant could sleep at the establishment rather than risk driving. Mr. Sergeant
stated he had everything fixed and corrected and had an inspection scheduled for
March 28, 2018. The Committee asked Ms. Temke to inform the Committee of the
inspections results.

11. 2018-0267 Discussion relative to the Food Peddler Permit and the food truck for Mister Bar
B-Q, Jervel M. Williams, agent for an incident occurring on March 11, 2018.

Appearance by Jervel M. Williams. Discussion ensued regarding the incident on
March 11, 2018 and Mr. Williams level of cooperation or lack thereof. Laura
Temke informed the Committee of the State Licensing for Food Trucks and the
City's policy for Food Trucks. Ms. Temke spoke of the numerous times West Allis
resources have been used to follow up with Mr. Williams and the food truck for
Mister Bar B-Q. The Committee asked Ms. Temke to provide them with the State
Inspection results once she receives it. The Committee asked about the plans Mr.
Williams has for the Boy Blue building and said they have heard good things
about his food, welcome his business in West Allis and asked that he cooperate
with the City requirements and respond to requests when he is contacted by City
Officials.

12. 2018-0265 Discussion relative to a memorandum from the Health Department Environmentalist
Coordinator for attempts to conduct inspections at Class B Tavern licensed
establishment, Premier Sports Lounge, Tabitha Harris, Agent, premises located at
1022 S. 60 St.

Nonappearance of Tabitha Harris. Ms. Harris contacted Laura Temke at the
Heath Department regarding not being able to appear. Ms. Temke informed the
Committee of the attempts made to do a Health Inspection. Ms. Temke said the
business has not been open when she does go there, and that it appears to not
have set or regular hours of business.
A motion was made by Ald. Weigel, seconded by Ald. Haass, that this matter was Placed on File.
The motion carried by the following vote:

13. 2018-0266 Discussion relative to a memorandum from the Health Department Environmentalist
Coordinator for attempts to conduct inspections, and the not yet issued Class B
Tavern License of Hawley House Restaurant, Marcia Thekan, Agent, premises
located at 1101 S. 60 St.

Nonappearance of Marcia Thekan. Ms. Thekan contacted the Clerk's Office.
Laura Temke informed the Committee of Ms. Thekan's nonoperations, the Class B
Tavern License not being issued within the sixty (60) day requirement period of
being approved, and the inability to conduct a Health Inspection of the premises.
A motion was made by Ald. Haass, seconded by Ald. Weigel, that this matter was Placed on File.
The motion carried by the following vote:
Aye: 5- Ald. Czaplewski, Ald. Roadt, Ald. Haass, Ald. May, and Ald. Weigel

F. ADJOURNMENT
A motion was made by Ald. Roadt, seconded by Ald. Haass, to adjourn at 7:12 p.m. The motion
carried unanimously.
