Office of the Board of Health City Hall 166 Boulder Drive – Suite 108 Fitchburg, Massachusetts 01420 978-829-1870 An organizational meeting was held on September 7, 2017 at 5:00 pm. Present: Ian Murray, Chairman; Stephen Curry, Director; Sandra Knipe, R.N.B.S.N., Member; Dr. John Bogdasarian, M.D, Member; Bob Proctor; Dennis Bradley; Tim Beauchemin P.E. Title 5 Local Upgrade Approval Request for 625 Sheldon Road: Bob Proctor will be the Installer and is here to present for Douglas Smith, Soilsmith Designs (unable to attend). The Installer and Engineer are seeking a Local Upgrade Approval (LUA). 1) Requesting a Local Upgrade Approval 310CMR 15.405(1)(h) Reduction of the requirement of a 4’separation between the bottom of the soil absorption system and high groundwater, with a class 2 soil having a reported perc rate of 12 minutes an inch. This results with the soil absorption system being 3’ above high groundwater. Dr. Bogdasarian made a motion to allow the 1 ft. LUA. Motion was seconded by Sandra Knipe. All were in favor. Motion carries. Title 5 Local Upgrade Approval Request for 190 Ashburnham Hill Road: Bob Proctor will be the Installer and is here to present for Douglas Smith, Soilsmith Designs (unable to attend). Sandra Knipe made a motion to approve the reduction of the requirement of a 5 ft. separation to a 3 ft. above high groundwater. Motion was seconded by Dr. Bogdasarian. All were in favor. Motion carries. Great Wolf Lodge – Request for Variance discussion: Mr. Curry stated Great Wolf Lodge (GWL) will be submitting a variance request to the BOH. GWL has several swimming pools in their facility and they are experiencing difficulty meeting the current standards of the Massachusetts Pool Code for Combined Chlorine. The chemical standard for Combined Chlorine presently in the code is 0.0-0.2 PPM (mg/L). GWL is requesting the BOH to allow a variance of this code to make 0.4 PPM (mg/L) an acceptable level. GWL has had several documented instances where the municipal water entering the facility tested a Combined Chlorine reading of 0.4 PPM (mg/L). This written request will most likely occur in the next few days and will appear on the October meeting agenda. Title 5 Local Upgrade Approval Request for 419 Ashburnham Hill Road: Tim Beauchemin P.E. is requesting 5 Local Upgrade Approvals on the property. 1) Requesting a 2 ft. groundwater offset that is allowed with a Presby System. 2) A Leaching system less than 20’ to the foundation 10 feet provided. 3) Septic tank less than 10’ to foundation, 3 feet provided. 4) Leaching system less than 100’ to private well, 85 feet provided. 5) One deep hole in disposal area instead of 2. Mr. Beauchemin stated the lot is 1.9 acres but is surrounded by ledge and other conditions on the property. Sandra Knipe suggested if the upgrade is approved to test the water more frequently. Mr. Curry suggested to test in one year. Sandra Knipe made a motion to approve the following LUA’S with the condition that well water testing occur within one year after construction and usage of the system: 1) The Leaching System is less than 20 ft. to the foundation and there is only 10ft. provided. 2) The septic tank is less than 10 ft. to the foundation, 3 feet provided. 3) The Leaching System is less than 100 ft. to the well with only 85 ft. provided (owner to provide a water sample one year from the date that the system is installed and approved by the Health Department). 4) The Leaching System is less than 4 ft. above groundwater, 2 ft. is provided. 5) One deep hole in the disposal area instead of 2. Dr. Bogdasarian seconded the motion. All were in favor. Motion carries. Cosmetic Tattoo Applicant Review: Dennis Bradley Mr. Curry stated Mr. Bradley’s experience pursuant to the BOH Body Art must be discussed. It has past practice that Cosmetic Body Art applicants present to the BOH The Fitchburg Rules and Regulations for Body Art Establishments states two years actual experience in the practice of performing body art activities. Mr. Bradley stated he would like to open a business for permanent cosmetics that would include permanent eyebrows and micro hair pigmentation. Mr. Bradley has a location on Summer St that he would be using for his practice. Mr. Curry asked for documentation for two years of experience. Mr. Bradley showed the Board Members a certificate from Connecticut in Body Art. He also stated he went to school in Florida and practiced there to learn the cosmetics. Ms. Knipe asked why Mr. Bradley has a permit to practice in Connecticut. Mr. Bradley stated he owns a building there and would eventually practice there as well. Mr. Bradley also stated he has a Medical License in the Dominican Republic that is equivalent in Massachusetts. Sandra Knipe made a motion to approve a Body Art Practitioner Permit for Mr. Bradley with the stipulation that no Practitioner Permit be issued until the Body Art Establishment is approved and licensed. This motion was seconded by Dr. Bogdasarian. All were in favor. Motion carries. August Meeting Minutes Review & Approval: Sandra Knipe made a motion to approve the August 3, 2017 meeting minutes. Motion was seconded by Dr. Bogdasarian. All were in favor. Motion carries. Staff Reports: Mr. Curry presented and discussed the Housing Inspectors Reports & Nurse’s Reports for January-August 2017. Sandra Knipe made a motion to approve the Housing Inspectors Reports January-August, 2017 as presented. Motion was 2nd by Dr. Bogdasarian. All were in favor. Motion carries. Sandra Knipe made a motion to approve the Nurses Report. Motion was seconded by Dr. Bogdasarian. All were in favor. Motion carries. Ian Murray made a motion to adjourn the meeting at 6:30 pm. Motion was seconded by Dr. Bogdasarian. All were in favor. Motion carries. Meeting adjourned at 6:30 pm.
