I. CALL TO ORDER
Chairman Tim O’Neill called the City of Henderson Self-Funded Insurance
Committee Regular meeting to order at 10:00 a.m., in the Mayor and Council
Boardroom, 4th Floor, City Hall, 240 Water Street, Henderson, Nevada.
II. CONFIRMATION OF POSTING AND ROLL CALL
Alysa Neilson, Recording Secretary, confirmed the Self-Funded Insurance
Committee meeting had been noticed in compliance with the Open Meeting Law
by posting the Agenda three working days prior to the meeting at City Hall, the
Multigenerational Center, the Whitney Ranch Recreation Center, Fire Station
No. 86, the Nevada Public Notice Website, the City of Henderson Website, and
by mailing a copy of the Agenda to everyone appearing thereon on the Agenda
Master Mailing List.

III. PUBLIC COMMENT
There were no comments presented by the public.
IV. ACCEPTANCE OF AGENDA
(Motion) Mr. Kerby introduced a motion to accept the agenda as
presented. The vote favoring approval was unanimous.
Chairman O’Neill declared the motion carried.
V. UNFINISHED BUSINESS
UB-1 SELF-FUNDED HEALTH PLAN DESIGN
HUMAN RESOURCES
Revise plan design effective January 1, 2017 for the purpose of creating plan
savings of at least $350,000.
Bob Osip, Risk Manager, introduced the item and gave a brief summary of the
revised plan design.
Discussion ensued regarding the decision to increase the out-of-pocket
maximum 25 percent and the costs versus benefits of increasing the
out-of-pocket maximum as well as other plan increases.
It was noted that Option 2 would be the least impactful concerning dental
coverage.
Jennifer moved to adopt Option 2 for the plan design option for Delta Dental
effective January 1 of 2017. Unanimous except Courtney and Kerby. Motion
carries.
(Motion) Ms. Fennema introduced a motion to adopt Option 2 for the plan
design, effective January 1, 2017. The vote favoring approval was
unanimous. Chairman O’Neill declared the motion carried.

(Motion) Ms. Fennema introduced a motion to adopt the August 10, 2016,
minutes as presented. The vote favoring approval was
unanimous. Chairman O’Neill declared the motion carried.
NB-3 MONTHLY FINANCIAL REPORT
HUMAN RESOURCES
Self-Funded Health Plan monthly financial report.
Maria Gamboa, Accounting Manager, reviewed the monthly financial reports
and noted the changes comparing this year’s numbers to last year’s numbers.
NB-4 LOOMIS MONTHLY CLAIMS REPORT
HUMAN RESOURCES
Loomis monthly claims report.
Bill Bixler, Loomis, reviewed the Loomis monthly claims report and noted the
high numbers are a result of high claims. He noted three large claims are
causing a significant increase in the total claims paid for the year.
NB-5 HEALTH PLAN OF NEVADA STATUS REPORT
HUMAN RESOURCES
Health Plan of Nevada status report.
Amber Hubber, Health Plan of Nevada, gave a summary of the Health Plan of
Nevada status report. She noted additional support and services are available
for members, including mobile medical center and urgent care home waiting
room.
NB-6 GALLAGHER BENEFIT SERVICES STATUS REPORT
HUMAN RESOURCES
Gallagher Benefit Services status report.
Shawn Adkins, Gallagher Benefit Services, gave a summary of the status report
and noted increases in the costs of mental health and substance abuse.
NB-7 SELF-FUNDED HEALTH INSURANCE PLAN RETIREE RATES

HUMAN RESOURCES
The 2017 renewal report establishing actuarial determined funding rates for
actives and retirees will be completed once all plan changes have been
confirmed for the new plan year beginning January 1, 2017. Request for
committee to accept the actuarial determined funding rates for the Self-Funded
Health Insurance plan for the purpose of establishing the retiree premiums.
Bob Osip, Risk Manager, note that this is on the agenda for the Committee to
approve in advance of the actual numbers or the Committee can wait for the
actual numbers to come in at the end of the year. He said the request is being
made in an effort to avoid an additional meeting.
It was the consensus of the Committee to wait for the year-end number prior to
approving retiree funding rates.
No action was taken on this item.
NB-8 HEALTH PLAN OF NEVADA MEDICAL INSURANCE PROPOSAL FOR PLAN
YEAR 2017
HUMAN RESOURCES
Review and accept the Health Plan of Nevada medical insurance contract
proposal for plan year 2017.
Amber Hubber, Health Plan of Nevada, gave a brief summary and noted the
offer is for the existing rates for twelve months for 2017.
Chairman O’Neill moved for approval. Unanimous.
(Motion) Chairman O’Neill introduced a motion to accept the Health Plan of
Nevada medical insurance contract for plan year 2017. The vote
favoring approval was unanimous. Chairman O’Neill declared the
motion carried.
NB-9 HEALTH PLAN OF NEVADA RETIREE RATES
HUMAN RESOURCES
The 2017 premiums for the Health Plan of Nevada plan will be confirmed once
the renewal request has been reviewed and approved by the Committee.
Request committee to allow staff to apply the rates in the renewal, once
adopted, for the purpose of establishing the Health Plan of Nevada retiree
premiums.
