BOARD OF PUBLIC WORKS AND SAFETY MINUTES June 28, 2016 Regular Session: The Board of Public Works and Safety met in regular session on Tuesday, June 28, 2016 at 9:00 a.m. in the Common Council Chambers. Members present were: Gary Henriott, Cindy Murray, Dawn Ross, Norm Childress and Ron Shriner. Jacque Chosnek, 1st Deputy City Attorney, was also present. President Henriott called the meeting to order. The Pledge of Allegiance was given to the flag of our Country. MINUTES Mrs. Murray moved for approval of the minutes from the June 21, 2016 regular meeting. Mrs. Ross seconded. Passed. QUOTE OPENING Curb Replacement for 7th Street, 8th Street and Oaklawn Drive This being the time set to open quotes for the Curb Replacement for 7th Street, 8th Street and Oaklawn Drive, the following quotes were received and opened: Rieth-Riley Construction Lafayette, Indiana Base Bid: $50,378.00 Dixon Construction Lafayette, Indiana Base Bid: $28,146.50 Mr. Childress moved to take the quotes under advisement for further review. Mr. Shriner seconded. Passed. BID OPENING Structure Demolition-608 S. 4th Street This being the time set to open bids for the Structure Demolition at 608 S. 4th Street, the following bids were received and opened: Hack Excavating Lafayette, Indiana Base Bid: $11,650.00 Board of Public Works and Safety June 28, 2016 Page 2 C&S Excavating Lafayette, Indiana Base Bid: $24,000.00 Yardberry Landscape Lafayette, Indiana Base Bid: $24,000.00 Mr. Shriner moved to take the bid under advisement for further review. Mrs. Ross seconded. Passed. NEW BUSINESS Economic Development Change Order #4-Main Street Streetscape Phase I & II Dennis Carson, Economic Development Director, presented to the Board and recommended approval of Change Order #4 for the Main Street Streetscape Phase I & II. The change order is an increase in the amount of $1,401.44 for Pavement Removal. Mrs. Murray moved for approval. Mrs. Ross seconded. Passed. Change Order #5-Main Street Streetscape Phase I & II Mr. Carson presented to the Board and recommended approval of Change Order #5 for the Main Street Streetscape Phase I & II. The change order is an increase in the amount of $14,607.01 for PVC Roof Drains. Mrs. Murray moved for approval. Mrs. Ross seconded. Passed. Change Order #6-Main Street Streetscape Phase I & II Mr. Carson presented to the Board and recommended approval of Change Order #6 for the Main Street Streetscape Phase I & II. The change order is an increase in the amount of $5,104.86 for Tree Receptacle Revision. Mr. Carson stated that Change Orders #4, #5 and #6 total $21,113.31 which brings the revised contract amount to $4,503,677.58. Mrs. Murray moved for approval. Mrs. Ross seconded. Passed. Lafayette Housing Authority Amendment #3-Agreement between Lafayette Housing Consortium and Lafayette Transitional Housing Center 2014-2015 Singles Program Rehab Valerie Oakley, Project Manager, presented to the Board and recommended approval of Amendment #3 for the Agreement between Lafayette Housing Consortium and Lafayette Transitional Housing Center 2014-2015 Singles Program Rehab. This amendment will allow them to finish the final paperwork and disburse the remaining funds through July 30, 2016. Mr. Childress moved for approval. Mrs. Murray seconded. Passed. President Henriott abstained. Board of Public Works and Safety June 28, 2016 Page 3 Amendment #1-Agreement between Lafayette Housing Consortium and Habitat for Humanity 2015-2016 Project #1-North 18th Street Mrs. Oakley presented to the Board and recommended approval of Amendment #1 for the Agreement between Lafayette Housing Consortium and Habitat for Humanity 2015-2016 Project #1 North 18th Street. This amendment will allow them to finish the final paperwork and disburse the remaining funds through July 30, 2016. Mr. Shriner moved for approval. Mrs. Ross seconded. Passed. Amendment #1-Agreement between Lafayette Housing Consortium and Faith Community Development Corp. 2014-2015 Project #3-North 8th Street Mrs. Oakley presented to the Board and recommended approval of Amendment #1 for the Agreement between Lafayette Housing Consortium and Faith Community Development Corp 2014-2015 Project #3- North 8th Street. This amendment will extend the ending date to October 30, 2016 to provide additional time should the current purchase offer not go through closing. Mrs. Murray moved for approval. Mrs. Ross seconded. Passed. Agreement between Lafayette Housing Consortium and Faith Community Development Corp 2015-2016 Project #1-Burroughs Mrs. Oakley presented to the Board and recommended approval of an Agreement between Lafayette Housing Consortium and Faith Community Development Corp 2015-2016 Project #1 Burroughs. This agreement is in the amount of $75,000.00 and is for the first of two projects under the 2015-2016 grant award to Faith CDC. The project will renovate and sell 1326 Burroughs Street to an eligible homebuyer. Mrs. Oakley answered questions from the Board about the selection process for these types of homes. Mrs. Ross moved for approval. Mrs. Shriner seconded. Passed. CLAIMS Mike Jones, Controller, presented for Board approval, Claims in the amount of $1,019,524.20. President Henriott asked if there were any further questions and there were none. Mrs. Murray moved for approval. Mr. Shriner seconded. Passed. MISCELLANEOUS Special Event Request-Crowe Wedding Anniversary Block Party President Henriott presented to the Board and recommended approval of a Special Event Request for the Crowe Wedding Anniversary Block Party to be held on July 2, 2016 from 2:00pm-2:00am at Colfax Street between N. 19th Street and N. 18th Street. President Henriott recommended amending the request to include a change in the fireworks and changing the time of the end of the party from 2:00am to 12:00midnight due to the ordinance. Mrs. Murray stated that the requester will be notified of the changes to the request. Mr. Shriner moved for approval of the amended request. Mrs. Ross seconded. Passed. Special Event Request-Zoo Run Run President Henriott presented to the Board and recommended approval of a Special Event Request for the Zoo Run Run to be held on July 16, 2016 from 7:30am-9:30am at Columbian Park and the surrounding neighborhood. Mr. Shriner moved for approval. Mrs. Ross seconded. Passed.
