BOARD OF PUBLIC WORKS AND SAFETY MINUTES October 9, 2018 Regular Session: The Board of Public Works and Safety met in regular session on Tuesday, October 9, 2018 at 9:00 a.m. in the Common Council Chambers. Members present were: Gary Henriott, Cindy Murray, Norm Childress and Amy Moulton. Absent: Ron Shriner. Jacque Chosnek, 1st Deputy City Attorney, was also present. President Henriott called the meeting to order. The Pledge of Allegiance was given to the flag of our Country. MINUTES Mr. Childress moved for approval of the minutes from the October 2, 2018 regular meeting. Mrs. Moulton seconded. Passed. NEW BUSINESS Parks Department Recommendation for Award- Tropicanoe Cove Aquatic Center Expansion Jon Miner, Parks Department, presented to the Board and recommended approval of a Recommendation for Award for the Tropicanoe Cove Aquatic Center Expansion with J.R. Kelly Company with a bid in the amount of $3,819,000.00. Mrs. Moulton moved for approval. Mrs. Murray seconded. Passed. Contract- Tropicanoe Cove Aquatic Center Expansion Mr. Miner presented to the Board and recommended approval of a Contract for the Tropicanoe Cove Aquatic Center Expansion with J.R. Kelly Company in the amount of $3,819,000.00. Mrs. Murray moved for approval. Mrs. Moulton seconded. Passed. Notice to Proceed- Tropicanoe Cove Aquatic Center Expansion Mr. Miner presented to the Board and recommended approval of a Notice to Proceed for the Tropicanoe Cove Aquatic Center Expansion to start today. The substantial completion date is set for April 16, 2019 with the final completion date set for before Memorial Day 2019. Mrs. Moulton moved for approval. Mr. Childress seconded. Passed. Change Order #3-Memorial Island Phase 2 Claudine Laufman, Parks Superintendent, presented to the Board and recommended approval of Change Order #3 for the Memorial Island Phase 2 with J.R. Kelly of a credit in the amount of $2,673.00 which brings the revised contract amount to $664,821.00. Mrs. Murray moved for approval. Mrs. Moulton seconded. Passed. Board of Public Works and Safety October 9, 2018 Page 2 Engineering Change Order #1-2018 Curb and Ramp Replacement Project S. 4th Street Jeromy Grenard, Public Works Director, presented to the Board and recommended approval of Change Order #1 for the 2018 Curb and Ramp Replacement Project on S. 4th Street with Dixon Construction. The change order is in the amount of $27,191.50 which brings the revised contract amount to $205,314.560. The change order includes a change in scope- sidewalk, curb and ramp added to 3rd Street. Mr. Childress moved for approval. Mrs. Moulton seconded. Passed. Lafayette Housing Authority CDBG Agreement between the City of Lafayette and Family Promise of Greater Lafayette, Inc. for a Family Emergency Homeless Shelter Program Seth Wells, Project Manager, presented to the Board and recommended approval of the CDBG Agreement between the City of Lafayette and Family Promise of Greater Lafayette, Inc. for a Family Emergency Homeless Shelter Program. The agreement is in the amount of $9,094.00. Mr. Childress moved for approval. Mrs. Moulton seconded. Passed. CDBG Agreement between the City of Lafayette and Lafayette Urban Ministry for an Emergency Homeless Shelter Program Year 2018 Mr. Wells presented to the Board and recommended approval of the CDBG Agreement between the City of Lafayette and Lafayette Urban Ministry for an Emergency Homeless Shelter Program Year 2018. The agreement is in the amount of $11,250.00. Mrs. Murray moved for approval. Mr. Childress seconded. Passed. CDBG Agreement between the City of Lafayette and YWCA of Greater Lafayette for the Domestic Violence Intervention and Prevention Program Year 2018 Mr. Wells presented to the Board and recommended approval the CDBG Agreement between the City of Lafayette and YWCA of Greater Lafayette for the Domestic Violence Intervention and Prevention Program Year 2018. The agreement is in the amount of $9,094.00. Mrs. Murray moved for approval. Mr. Childress seconded. Passed. CDBG Agreement between the City of Lafayette and Tippecanoe County Council on Aging, Inc. DBA Tippecanoe Senior Center for the Senior Home Repair Program Year 2018 Mr. Wells presented to the Board and recommended approval of the CDBG Agreement between the City of Lafayette of Tippecanoe County Council on Aging, Inc. DBA Tippecanoe Senior Center for the Senior Home Repair Program Year 2018. The agreement is in the amount of $50,000.00. Mr. Childress moved for approval. Mrs. Murray seconded. Passed. Board of Public Works and Safety October 9, 2018 Page 3 Purchasing Declaration of Worthless Property-IT Department Mike Jones, Controller, presented to the Board and recommended approval of a Declaration of Worthless Property for the IT Department that includes a list of equipment from IT. The equipment is no longer in service and will be recycled. Mrs. Moulton moved for approval. Mrs. Murray seconded. Passed. Declaration of Worthless Property-Police Department Mr. Jones presented to the Board and recommended approval of a Declaration of Worthless Property for the Police Department that includes a 2013 Black Dodge Charger 4-Dr Sedan VIN: 2C3CDXAGXDH546377 City#13-212. The vehicle has been declared a total loss after a crash and will be sold as salvage. Mr. Childress moved for approval. Mrs. Moulton seconded. Passed. Declaration of Worthless Property-Engineering Department Mr. Jones presented to the Board and recommended approval of a Declaration of Worthless Property for the Engineering Department that includes a Panasonic PV-A386D Palmcorder IQ VHS Camcorder Serial Number H6WA10827 with tapes and accessories. This item will be recycled. Mrs. Moulton moved for approval. Mrs. Murray seconded. Passed. Lafayette Renew Certificate of Substantial Completion-Brown Street Sewer Project Brad Talley, Renew Superintendent, presented to the Board and recommended approval of a Certificate of Substantial Completion for Brown Street Sewer Project. The contractor Bowen Engineering has completed all the work for this project with a substantial completion date of February 7, 2018. Mrs. Murray moved for approval. Mr. Childress seconded. Passed. Certificate of Final Completion-Brown Street Sewer Project Mr. Talley presented to the Board and recommended approval of a Certificate of Final Completion for the Brown Street Sewer Project. The contractor Bowen Engineering has completed all the work for this project with a final completion date of May 22, 2018. Mrs. Moulton moved for approval. Mr. Childress seconded. Passed. Significant Industrial User Permit-Cargill, Inc Brian Beeler, Lafayette Renew, presented to the Board and recommended approval of a Significant Industrial User Permit for Cargill, Inc. Cargill has a 4-year permit through the pre-treatment program. Mr. Childress moved for approval. Mrs. Moulton seconded. Passed. Significant Industrial User Permit-Caterpillar, Inc Mr. Beeler presented to the Board and recommended approval of a Significant Industrial User Permit for Caterpillar, Inc. Caterpillar has a 4-year permit through the pre-treatment program. Mrs. Murray moved for approval. Mr. Childress seconded. Passed. Board of Public Works and Safety October 9, 2018 Page 4 CLAIMS Mr. Jones presented for Board approval, Claims in the amount of $383,694.98. President Henriott asked if there were any further questions and there were none. Mr. Childress moved for approval. Mrs. Moulton seconded. Passed. MISCELLANEOUS 2019 Holiday Schedule Mrs. Murray presented to the Board and recommended approval of the 2019 Holiday Schedule. This schedule has been looked over by the Mayor’s Office, Clerk’s Office and Human Resources. Mr. Childress moved for approval. Mrs. Moulton seconded. Passed. Special Event Request-2018 Christmas Parade Mrs. Murray presented to the Board and recommended approval of a Special Event for the 2018 Christmas Parade to be held on Saturday, December 1, 2018 in downtown Lafayette. The closures will begin around 12:00pm that day. Mr. Childress moved for approval. Mrs. Moulton seconded. Passed. Special Event Request-Tis the Season 5K Holiday Run President Henriott presented to the Board and recommended approval of a Special Event Request for the Tis the Season 5K Holiday Run to be held on December 1, 2018 from 9:00am-10:15am in downtown Lafayette. Mrs. Murray moved for approval. Mr. Childress seconded. Passed. Proclamation-Mental Illness Awareness Week 2018 Mayor Roswarski read the following proclamation: WHEREAS: Mental health is part of overall health, and; WHEREAS: One in five adults experience a mental health problem in any given year, and; WHEREAS: Approximately one-half of chronic mental illness begins by the age of 14 and three- quarters by the age 24; and, WHEREAS: Suicide is the 10th leading cause of death in the United States and the second leading cause among young adults, and 90% of people who die by suicide have an underlying mental illness; and, WHEREAS: Long delays – sometimes decades – often occur between the time symptoms first appear and when individuals get help; and, WHEREAS: Early identification and treatment can make a difference in successful management of mental illness and recovery; and, WHEREAS: It is important to maintain mental health and learn the symptoms of mental illness in order to get help when it is needed; and Board of Public Works and Safety October 9, 2018 Page 5 WHEREAS: Every citizen and community can make a difference in helping end the silence and stigma that for too long has surrounded mental illness and discouraged people from getting help; and, WHEREAS: Public education and civic activities can encourage mental health and help improve the lives of individuals and families affected by mental illness. NOW, THEREFORE BE IT RESOLVED, I, Tony Roswarski, Mayor of Lafayette, Indiana, do hereby proclaim October 7 through October 13, 2018 as Mental Illness Awareness Week in Lafayette, Indiana, to shine a light on mental illness and fight stigma, provide support, educate the public and advocate for equal care.
