Mr. D’Andria’s pension is calculated for an annual amount of $7,925.43. Mr. D’Andria’s application for a deferred retirement was approved at the 4/19/2011 meeting. H. Notification of death of surviving spouse Monique Veronick (DOD 11/24/2011), who’s husband retired from the Golf Authority on 5/6/1993, no survivorship benefit due. I.
