CALL TO ORDER The meeting was called to order at 5:30 p.m. 1.A ROLL CALL Present: 5 - Chair Jim Cooper, Vice Chair Jeannine Roe, Boardmember Russ Gies, Boardmember Caleb Martin, and Boardmember Jack Seward 2. APPROVAL OF MINUTES 2.A 14-0935 Approval of July 14, 2014 LEOFF Disability Board Meeting Minutes Boardmember Seward moved, seconded by Vice Chair Roe, to approve the minutes. The motion carried by the following vote: Aye: 4 - Chair Cooper, Vice Chair Roe, Boardmember Gies and Boardmember Seward 3. BUSINESS ITEMS 3.A 14-0936 LEOFF Disability Board Public-at-Large Interview Boardmember Seward moved, seconded by Vice Chair Roe, to approve appointing Caleb Martin to the Member-at-Large position on the LEOFF Disability board. The motion was approved by the following vote: Aye: 4 - Chair Cooper, Vice Chair Roe, Boardmember Gies and Boardmember Seward 3.B 14-0937 Case #6-14 Hearing Aid Request Boardmember Seward moved, seconded by Vice Chair Roe, to approve the request for hearing aids in the amount of $5,500. The motion carried by the following vote: Aye: 5 - Chair Cooper, Vice Chair Roe, Boardmember Gies, Boardmember Martin and Boardmember Seward 3.C 14-0938 Case #7-14 Dental Request City of Olympia Page 1 LEOFF I Disability Board Meeting Minutes October 13, 2014 Vice Chair Roe moved, seconded by Boardmember Martin, to approve the dental request in the amount of $2,246. The motion carried by the following vote: Aye: 5 - Chair Cooper, Vice Chair Roe, Boardmember Gies, Boardmember Martin and Boardmember Seward 3.D 14-0987 Case #08-14 Dental Reimbursement Request Boardmember Seward moved, seconded by Vice Chair Roe, to approve the dental reimbursement request for any out-of-pocket expenses not covered by the member's Group Health insurance not to exceed $7,844. The motion carried by the following vote: Aye: 5 - Chair Cooper, Vice Chair Roe, Boardmember Gies, Boardmember Martin and Boardmember Seward 3.E 14-0939 MEDPlus Hearing Aid Provider The Board was given information regarding the MedPlus Hearing Aid program, which included a list of providers. The report was received. 4. REPORTS 4.A 14-0940 LEOFF 1 Disability Board Expense Reports The August, 2014 expense report was distributed and reviewed by Boardmembers. The information was provided. 5.
