CALL TO ORDER Chair Cooper called the meeting to order at 5:30 p.m. 2. APPROVAL OF MINUTES 3.A 15-0587 Approval of May 11,2015 LEOFF Disability Board Meeting Minutes Boardmember Seward moved, seconded by Boardmember Martin, to approve the minutes. The motion carried by the following vote: Aye: 4 - Chair Cooper, Vice Chair Roe, Boardmember Martin and Boardmember Seward Excused: 1 - Boardmember Gies 4. BUSINESS ITEMS 4.A 15-0565 Case #03-15 Hearing Aid Request Boardmember Seward moved, seconded by Boardmember Martin, to approve the purchase of hearing aids in the amount of $5,400. The motion carried by the following vote: Aye: 4 - Chair Cooper, Vice Chair Roe, Boardmember Martin and Boardmember Seward Excused: 1 - Boardmember Gies City of Olympia Page 1 LEOFF I Disability Board Meeting Minutes June 8, 2015 4.B 15-0567 Case #04-15 Dental Request No decision was made by the Board pending LEOFF member submission from a medical doctor showing medical necessity for the dental work that was performed. 4.C 15-0468 Hearing Aid Procedure The decision was discussed, and the Board decided to rewrite the proposed language to be decided on at the August 10, 2015 LEOFF I Disability Board meeting under File 15-0767. 4.D 15-0469 Dental Care Rule Revision Boardmember Roe moved, seconded by Boardmember Martin, to approve the Dental care revision with minor revisions as follows: Bolded “are actively employed” in the first paragraph and bolded “are not actively employed” in the second paragraph. The motion carried by the following vote: Aye: 4 - Chair Cooper, Vice Chair Roe, Boardmember Martin and Boardmember Seward Excused: 1 - Boardmember Gies 5. REPORTS 5.A 15-0563 Expense Report for April 2015 The expense report for April 2015 was reviewed and discussed by the Board. This is an informational item and no action was required.
