CALL TO ORDER Chair Cooper called the meeting to order at 5:30 p.m. 2 ROLL CALL Present: 4 - Chair Jim Cooper, Boardmember Russ Gies, Boardmember Caleb Martin, and Boardmember Jack Seward Excused: 1 - Vice Chair Jeannine Roe 2.A OTHERS PRESENT Carl Watts, Personnel Analyst 3. APPROVAL OF MINUTES 3.A 15-0765 Approval of June, 2015 LEOFF Disability Board Meeting Minutes The minutes were approved. 4. BUSINESS ITEMS 4.A 15-0766 Case #05-15 Dental Request Boardmember Martin moved, seconded by Boardmember Gies, to approve payment for Case 05-15 for dental services in the amount of $3,736. The motion carried by the following vote: Aye: 4 - Chair Cooper, Boardmember Gies, Boardmember Martin and Boardmember Seward Excused: 1 - Vice Chair Roe 4.B 15-0768 Case #06-15 Long Term Care Boardmember Seward moved, seconded by Boardmember Martin, to approve Case #06-15 Long Term Care request in the amount of $1,186.50 for service through July 31, 2015. This is a one-time exception, because the provider used was not licensed and bonded, but the member did not realize this. The motion carried by the following vote: Aye: 4 - Chair Cooper, Boardmember Gies, Boardmember Martin and Boardmember Seward Excused: 1 - Vice Chair Roe 4.C 15-0767 Hearing Aid Procedure Rule Revision Boardmember Seward moved, seconded by Boardmember Martin, to change the proposed language for hearing aid procedure in paragraph two from “Member preference will not be one of the deciding factors in the Board’s decision on which of the two quotes to accept,” to “The final decision will be at the discretion of the Board." The motion carried by the following vote: Aye: 4 - Chair Cooper, Boardmember Gies, Boardmember Martin and Boardmember Seward Excused: 1 - Vice Chair Roe 5. REPORTS 5.A 15-0769 Expense Report-June 2015 The expense report for June 2015 was received. This is an informational item and no action was required. 6.
