CALL TO ORDER Chair Cooper called the meeting to order at 5:30 p.m. 1.A ROLL CALL Present: 5 - Chair Jim Cooper, Vice Chair Jeannine Roe, Boardmember Russ Gies, Boardmember Caleb Martin, and Boardmember Jack Seward OTHERS PRESENT Personnel Analyst Carl Watts 3. APPROVAL OF MINUTES 3.A 15-0959 Approval of August 10, 2015 Minutes The minutes were approved. 4. BUSINESS ITEMS 4.A 15-0960 Case # 07-15 Hearing Aid Request Boardmember Roe moved, seconded by Boardmember Martin, to approve the hearing aid request in the amount of $5,200. The motion was unanimously approved. 4.B 15-0961 Case #08-15 Long Term Care No specific amount was determined, but the estimate was below the $290/day in the LEOFF policies at $213.33/day after the one-time community fee of $2,000 was paid. Boardmember Seward moved, seconded by Boardmember Martin, to approve the long-term care request and the community fee. The board asked staff to contact Ascent audiology one more time to see if they would offer a discount. The motion was unanimously approved. 5.
