The meeting was called to order at 5:03 p.m.

2. APPROVAL OF MINUTES
2.A 21-0983 Approval of September 13, 2021 LEOFF Board Minutes
The minutes were approved.
3. BUSINESS ITEMS
3.A 21-0984 Approval of Case 21-9 Dental Request
Boardmember Dibble moved, seconded by Boardmember Gies, to approve
the request for case #21-9 in the amount of $2,216.00. The motion carried by
the following vote:
Aye: 4 - Chair Cooper, Boardmember Dibble, Boardmember Gies and
Boardmember Selby
Excused: 1 - Vice Chair Hansen
