MINUTES Sand Springs Firefighters Pension Board Sand Springs Municipal Building 100 East Broadway - Room 201 Sand Springs, Oklahoma 74063 June 16, 2011 - 8:30 a.m. The Firefighters Pension Board met in special session on June 16, 2011 in Room #201 of the Sand Springs Municipal Building, pursuant to the special meeting notice and agenda filed with the City Clerk’s office and posted at 4:10 p.m, on June 13, 2011 on the bulletin board located in the first floor lobby of the Sand Springs Municipal Building, 100 East Broadway, Sand Springs, Oklahoma, 74063. 1. Call to Order The meeting was called to order at 8:32 a.m. 2. Roll Call It was noted for the records that Justin Melton, Jeremy Wade, and Janice Almy were in attendance. 3. Minutes A motion was made by Justin Melton and seconded by Jeremy Wade that the minutes of the April 3, 2008 special Firefighters Pension Board meeting, as presented, be approved. Ayes: Melton, Wade, Almy Nay: None 4. Application of Death Benefit and Surviving Spouse for Pension Mary Lea Forsythe, spouse of former Firefighter W.W. “Bill” Forsythe, has submitted an Application for Death Benefit and Surviving Spouse for Pension. Following review of the applications, a motion was made by Jeremy Wade and seconded by Justin Melton that the requested approval of Application of Death Benefit and Surviving Spouse for Pension, as presented, be approved.
