MINUTES Sand Springs Firefighters Pension Board Sand Springs Municipal Building 100 East Broadway - Administration Sand Springs, Oklahoma 74063 May 11, 2012 - 8:30 a.m. The Firefighters Pension Board met in special session on May 11, 2012 in Administration offices of the Sand Springs Municipal Building, pursuant to the special meeting notice and agenda filed with the City Clerk’s office and posted at 4:45 p.m, on May 7, 2012 on the bulletin board located in the first floor lobby of the Sand Springs Municipal Building, 100 East Broadway, Sand Springs, Oklahoma, 74063. 1. Call to Order The meeting was called to order at 9:18 a.m. by Jeremy Wade 2. Roll Call It was noted for the records that Jeremy Wade, Stephen Brewer and Janice Almy, as well as Phillip Stephens were in attendance. 3. Minutes A motion was made by Jeremy Wade and seconded by Stephen Brewer that the minutes of the August 30, 2011 special Firefighters Pension Board meeting, as presented, be approved. Ayes: Wade, Brewer, Almy Nay: None 4. Application of Death Benefits and for Surviving Spouse Pension Nola J. Durkee submitted an Application of Death Benefits and Surviving Spouse Pension. Following review of the application and discussion, a motion was made by Jeremy Wade and seconded by Stephen Brewer that the requested approval of Application of Death Benefits and for Surviving Spouse Pension, as presented, be approved.
