The meeting was called to order by Chair Czaplewski at 6:00 p.m.

C. APPROVAL OF MINUTES
2014-0738 Minutes (Draft) of the License & Health special meeting of November 18, 2014
and recess meeting of November 18, 2014.

A motion was made by Ald. Roadt, seconded by Ald. Haass, that this matter be Approved.
The motion carried unanimously.
All in attendance were told of the importance of calling 911 for any signs of
trouble, encouraged to call and to be helpful and cooperative with the police
when they arrive.
They were told to make good choices when serving patrons and that it is illegal
to over serve or serve someone already intoxicated.
They were informed of the City's Compliance Check Programs and to check
identification.
They were reminded that a licensed bartender must be on duty during all hours
of operation.
The Class B Tavern License applicants were told that they must buy beer and
alcohol from a distributor.

31. 2014-0743 2014-2015 Class B Tavern License application no. 2156 of Papa’s Pub, LLC,
Dale J. Kristbaum, Agent, d/b/a Papa’s Pub, for premises located at 1101 S. 60
St. (new-existing location).
Appearance by Dale J. and Linda Kristbaum. Discussion ensued regarding Mr.
Kristbaum's experience in running a tavern. He informed the Committee of Ms.
Kristbaum's experience and that she would be the manager.
This matter was Recommended for Approval subject to the necessary requirement(s).

32. 2014-0747 2014-2015 Class B Tavern License application no. 2159 of Riderz, LLC, Rafael
P. Quintero, Agent, d/b/a Tailgaters Sports Pub, for premises located at 6404 W.
National Ave. (new-existing location).
Appearance by Rafael P. Quintero. Discusssion ensued regarding Mr.
Quintero's experience in owning and running a tavern. Mr. Quintero asked the
Committee if he could change the d/b/a name to Tailgaters Sports Bar. Chair
Czaplewski informed him to write a letter regarding the name change and
submit it to the Clerk's Office.
This matter was Recommended for Approval as amended and subject to the necessary
requirement(s).

the Clerk's Office.
This matter was Recommended for Denial based on two (2) nonappearances..

Angela N. Griffin.
Appearance by Angela N. Griffin. Discusssion ensued regarding Ms. Griffin's
police report.
A motion was made by Ald. Roadt, seconded by Ald. Haass, that this matter was
Recommended For Approval. The motion carried by the following vote:

Shamsher S. Randhawa.
Appearance by Shamsher S. Randhawa. Discusssion ensued regarding Mr.
Randhawa's police report and the letter he submitted.
This matter was Recommended for Denial based on the police report.

36. 2014-0740 2014-2015 Operator’s License (bartender/sales clerk) application no. 15-527 for
Erin M. Bartes.
Appearance by Erin M. Bartes. Discusssion ensued regarding Ms. Bartes's
police report. Ms. Bartes was reminded to make good choices for the customers
and herself to not overserve.
This matter was Recommended For Approval.

37. 2014-0720 Police Report involving Six Points Pub & Grille, 6200 W. Greenfield Ave.
(2014-2015 Class B Tavern License no. 863, Dance Hall License no. 865, and
Instrumental Music License no. 866 of Escape Horse Inn, LLC, Douglas M.
Ross, Agent, Incident Date: November 1, 2014).
Appearance by Douglas M. Ross. Discussion ensued regarding the police
report. Mr. Ross apologized to the Police Department and the Committee for
the way he came across in the police report. He meant no disrespect toward the
officers or the Police Department. Ald. Vitale commented he received a call
from Mr. Ross after the incident with concerns about the incident and the
patrons parking. Mr. Ross was advised to speak to Public Works regarding
additional parking limitation signs, such as no parking or motorcycle parking
only.
This matter was Recommended to be Placed on File.

Date: October 13, 2014).
Nonappearance of Lindsey Anne Guillermo. Ms. Guillermo contacted the
Clerk's Office. The Committee requested she be ordered to appear at the next
scheduled License & Health meeting.
Nonappearance of Hector Guillermo. Mr. Guillermo did not contact the Clerk's
Office. The Committee requested he be ordered to appear at the next scheduled
License & Health meeting.

40. 2014-0077 Amended Findings and Recommendations in the Matter of the Complaint
Against Scardina Bakery & Delicatessen, Inc., d/b/a Scardina Bakery &
Delicatessen, for premises located at 5901 W. Beloit Rd. (2013-2014 Food
Permit renewal application).

Appearance by Dorene and John Monty. Discussion ensued regarding the
delinquent taxes. The Committee directed the City Attorney to prepare Findings
and Recommendations for the Council meeting of December 16, 2014 with a
January deadline payment date.
This matter was Recommended for Nonrenewal and To issue a Findings and
Recommendation on a Block Vote.
Passed The Block Vote
A motion was made by Ald. Weigel, seconded by Ald. Roadt, to approve all the actions
on the preceding items on a Block Vote. The motion carried by the following vote:

Lincoln Ave.; Jets Pizza, 9129 W. Greenfield Ave.; Marv’s Gold Coast
Subs, 5901 W. Burnham St.; Sacred Skin, 9509 W. Greenfield Ave.;
Sammy’s, 10534 W. Greenfield Ave.; dated November 20, 2014.

Nonappearances of Anthony Volpe, Coldstone Creamery and Marcello
Orlando, Sacred Skin. They did not contact the Clerk's Office. The Committee

Nonappearance of Lynette Weidner, Cat Network. Ms. Weidner contacted the
City Attorney's Office and paid her delinquent personal property taxes in full.

Nonappearance of Timothy Sanchez, Fratelli Restaurant. Mr. Sanchez signed a
payment agreement with the City Attorney's Office for the delinquent personal
property taxes.
Nonappearance of William Waple, Jet's Pizza. Mr. Waple contacted the Clerk's
Office and will be paying the delinqunet personal property taxes in full.
Nonappearance of Rodney Waters, Gold Coast Subs. Mr. Waters contacted the
City Attorney's Office. Attorney Merten informed the Committee Mr. Waters is
a new owner and therfore not responsible for the delinquent personal property
taxes.
Nonappearance of Sammy Bohringer, Sammy's. Mr. Bohringer signed a
payment agreement with the City Attorney's Office for the delinquent personal
property taxes.
This matter was Held.
45. 2014-0751 Communication from Assistant City Attorney regarding Outstanding
Municipal Court Judgment issued against Golden Chicken, 8901 W.
Greenfield Ave., James Foti, (2014-2015 Food Permit/License).

Appearance by James Foti, Golden Chicken. Discussion ensued regarding the
Municipal Judgment. Mr. Foti was informed that no appearance would be
needed in January if this matter is taken care of and paid in full.
This matter was Held.
E. DISCUSSION ITEM
2014-0760 Discussion relative to late fee for liquor, beer and wine license renewal
applications.

2014 Survey Late Filing Fee
Discussion ensued regarding applying a fee and the amount to late renewal
applications. Monica Schultz, City Clerk, informed the Committee of other
Municipalities fees. The Committee directed the City Attorney to prepare an
Ordinance regarding a late fee of $100 for Class B Tavern License renewal
applications and Combination Class A Liquor License renewal applications and
a fee of $50 for Beer License renewal applications and Wine License renewal
applications. The ordinance would be mailed to license holders with a letter to
inform them once the ordinance is passed.
This matter was Discussed and Consensus Given.

All meetings of the License & Health Committee are public meetings. In order for the general public to make to comments at the committee meetings, the
individual(s) must be scheduled (as an appearance) with the chair of the committee or the appropriate staff contact; otherwise, the meeting of the
committee is a working seesion for the committees itself, and discussion by those in attendance is limited to committee members, the mayor, other
alderpersons, staff and others that may be a party to the matter being discussed.
