The meeting was called to order by Chair Czaplewski at 6:15 p.m.

Jennifer A. Falendysz, Lt. Jessica Johnson, West Allis Police Dept. and Ann
Marie Neff, Secretary to the Committee.
C. APPROVAL OF MINUTES
2014-0774 Minutes (draft) of the License & Health special meeting of December 2, 2014
and recess meeting of December 2, 2014.

A motion was made by Ald. Barczak, seconded by Ald. Haass, to approve the minutes of
the December 2, 2014 Special meeting and December 2, 2014 recess meeting. The motion
carried unanimously.
All in attendance were told of the importance of calling 911, encouraged to call
for any signs of trouble and to be helpful and cooperative with the police when
they arrive. They were reminded it is illegal to over serve or serve intoxicated
patrons. They were informed of the City's Compliance Check Programs and to
check identification.

D. NEW AND PREVIOUS MATTERS
64. 2014-0767 2014-2015 Class B Tavern License application no. 2165 of BHP Partners, LLC,
Bakul V. Patel, Agent, d/b/a Town Beer & Liquor, for premises located at 907
S. 84 St. (new-existing location).
Appearance by Bakul V. Patel. Discussion ensued regarding Mr. Patel's
experience with running a tavern and liquor store. He was encouraged to
develop a policy regarding employees drinking while working and reminded to
check identification and if there is doubt of the ID being valid to hold onto it
and call the police.
A motion was made by Ald. Barczak, seconded by Ald. Weigel to amended the
Public Entertainment Form to include the maximum of five (5) amusement
devices. The motion carried unanimously.
This matter was Recommended for Approval as amended and subject to the necessary
requirement(s) on a Block Vote.
65. 2014-0768 2014-2015 Combination Class A Liquor License application no. 2164 of BHP
Partners, LLC, Bakul V. Patel, Agent, d/b/a Town Beer & Liquor, for premises
located at 903 S. 84 St. (new-existing location).

This matter was Recommended for Approval subject to the necessary requirement(s) on a
Block Vote.
Passed The Block Vote
A motion was made by Ald. Barczak, seconded by Ald. Weigel, to approve all the actions
on the preceding items on a Block Vote. The motion carried by the following vote:

contact the Clerk's Office. The Committee requested she be ordered to appear
at the next scheduled License & Health meeting.

Durell L. Miller.
Nonappearance of Durell L. Miller. Ms. Miller did not contact the Clerk's

Class B Tavern License no. 1743 and Instrumental Music License no. 1788 of
Beer Garden Entertainment LLC, Lindsey Anne Guillermo. Agent, Incident
Date: October 13, 2014).
Nonappearance of Lindsey Anne Guillermo. Ms. Guillermo contacted the
Clerk's Office. She was represented by Attorney Patrick Hessling. Discussion
ensued regarding Ms. Guillermo's absence with regard to the management of
The Drunken Duck. Attorney Hessling could not tell the Committee how long
Ms. Guillermo has been out of town or how long she plans to be out of town, or
if she is looking at appointing a new agent. The Committee asked that Ms.
Guillermo be ordered to appear during recess at the Common Council meeting
of January 6, 2015 or that her Attorney appear with a written statement from
Ms. Guillermo regarding the management and the police report incidents.

have entered into a payment plan agreement with the City Attorney.
A motion was made by Ald. Weigel, seconded by Ald. Barczak, that this matter was
Referred for Summons and Complaint to the City Attorney for Cold Stone Creamery. The
motion carried by the following vote:

All meetings of the License & Health Committee are public meetings. In order for the general public to make to comments at the committee meetings, the
individual(s) must be scheduled (as an appearance) with the chair of the committee or the appropriate staff contact; otherwise, the meeting of the
committee is a working seesion for the committees itself, and discussion by those in attendance is limited to committee members, the mayor, other
alderpersons, staff and others that may be a party to the matter being discussed.
