A. CALL TO ORDER
The meeting was called to order by Chair Czaplewski at 6:00 p.m.
B. ROLL CALL

Seiy, Montal L. Hinton, Tonja M. Bassett, Janice Susan Fazen, Lt. Brian Saftig, West
Allis Police Dept., Nicholas Cerwin, Assistant City Attorney and Ann Marie Neff,
Secretary to the Committee.
C. APPROVAL OF MINUTES
1. 2017-0168 Minutes (draft) of the License & Health regular meeting of February 28, 2017,
recess meetings of March 7. 2017 and March 21, 2017.

A motion was made by Ald. Roadt, seconded by Ald. May, that this matter be Approved.
The motion carried unanimously.
D. NEW AND PREVIOUS MATTERS
Ald. Weigel informed all applicants in attendance to call the police when there are
incidents in the establishment, to be a partner with the police, to cooperate, to be
helpful and give details to the police, to follow the law, to not overserve patrons, that it
is illegal to serve someone that is already intoxicated, and of the City's Compliance
Check Programs.

2. 2017-0117 2016-2017 Adult-Oriented Establishment Operator Permit application no. 2410
of Willette L. Honey.
Appearance by Willette L. Honey. Ms. Honey stated she did not need the permit
because she no longer worked at the establishment.
A motion was made by Ald. Roadt, seconded by Ald. May, that this matter was
Recommended to be Placed on File. The motion carried by the following vote:
Aye: 5- Ald. Czaplewski, Ald. Roadt, Ald. Haass, Ald. May, and Ald. Weigel
No: 0
3. 2017-0170 2016-2017 Operator’s License (bartender/sales clerk) application no. 17-564 of
Donovan J. Kelly.
Appearance by Donovan J. Kelly. Discussion ensued regarding Mr. Kelly's police
report and the violations he did not list on his application. Mr. Kelly was advised to be
thorough when filling out applications in the future.
A motion was made by Ald. Haass, seconded by Ald. Roadt, that this matter was
Recommended For Approval. The motion carried by the following vote:
Aye: 4- Ald. Czaplewski, Ald. Roadt, Ald. Haass, and Ald. May

4. 2017-0159 2016-2017 Operator’s License (bartender/sales clerk) application no. 17-562 of
Sophia L. Ludwig.
Appearance by Sophia L. Ludwig. Discussion ensued regarding Ms. Ludwig's police
report, her driving violations, and her incomplete application. Ms. Ludwig was told
that her driver's record is important because it comes with rules and regulations to
follow and so does a bartender/sales clerk's license. She was advised to fill out
applications completely in the future.
A motion was made by Ald. Haass, seconded by Ald. May, that this matter was
Recommended For Approval. The motion carried by the following vote:
Aye: 3- Ald. Czaplewski, Ald. Roadt, and Ald. Haass
No: 2- Ald. May, and Ald. Weigel
5. 2017-0153 Police Department Report involving The Alibi Tavern, 6540 W. Grant St., Alibi
Taverns LLC, Patrick R. Kosidowski, Agent, Incident Date: February 22, 2017.
Nonappearance by Patrick R. Kosidowski. Mr. Kosidowski did not contact the Clerk's
Office. The Committee requested he be ordered to appear at the next scheduled
License & Health meeting.

6. 2017-0141 Police Department Report involving The Hideout, 6201 W. Mitchell St., of
Hideout West Allis, Inc., Daniel R. Seiy, Agent, Incident Date: February 16,
2017.
Appearance by Daniel R. Seiy. Discussion ensued regarding the police report, Mr.
Seiy's lack of cooperation with the Police Officers, and his level of intoxication while
operating his establishment. The Committee directed the City Attorney to issue a
Summons and Complaint for revocation or suspension of the licenses.
A motion was made by Ald. May, seconded by Ald. Haass, that this matter was Referred
for Summons and Complaint to the City Attorney. The motion carried by the following
vote:
Aye: 5- Ald. Czaplewski, Ald. Roadt, Ald. Haass, Ald. May, and Ald. Weigel
No: 0
7. 2017-0169 Communication from Montal L. Hinton regarding ownership of JM 6500
Investments, LLC, 2016-2017 Class B Tavern License no. 2335, for the
premises located at 6500 W. Greenfield Ave.
Appearance by Montal L. Hinton and Tonja M. Bassett. Discussion ensued regarding
ownership of JM 6500 Investments, LLC. Mr. Hinton and Ms. Bassett both provided
their own lease with their own name on it. City Attorney Cerwin stated the Committee
could file a complaint and have a hearing to determine if the licenses were
fraudulently applied for.
A motion was made by Ald. Roadt, seconded by Ald. Haass, that this matter was Referred
for Summons and Complaint to the City Attorney. The motion carried by the following
vote:

8. 2017-0142 Police Department Report involving 6500 Restaurant & Bar, 6500 W.
Greenfield Ave., of JM 6500 Investments, LLC, Tonja M. Bassett, Agent,
Incident Date: February 3, 2017.
Appearance by Tonja M. Bassett. Discussion ensued regarding the police report.
This matter was Held.
9. 2017-0151 Police Department Report involving 6500 Restaurant & Bar, 6500 W.
Greenfield Ave., of JM 6500 Investments, LLC, Tonja M. Bassett, Agent,
Incident Date: February 9, 2017.
Appearance by Tonja M. Bassett. Discussion ensued regarding the police report. Ald.
Czaplewski informed Ms. Bassett that he received a call regarding the trash receptacle
overflowing at the establishment and that she needs to have a current contract with the
company picking up the trash. Ms. Bassett replied she did and that someone had
dumped remodeling debris in their dumpsters which caused the issue.
A motion was made by Ald. Haass, seconded by Ald. Weigel, that this matter was Placed
on File. The motion carried by the following vote:

10. 2017-0171 Summons and Complaint in the matter of the complaint against Janice Susan
LLC, Janice Susan Fazen, Agent, for the premises located at 1641 S. 68 St.,
d/b/a Dog House (2016-2017 Class B Tavern License no. 1759 and 2016-2017
Instrumental Music License no. 1761).
Attachments: S&C Dog House 032816
Appearance by Janice Susan Fazen. Ms. Fazen supplied the necessary documents to
obtain her 2016-2017 licenses.
A motion was made by Ald. Roadt, seconded by Ald. Weigel, that this matter be Placed
on File. No vote was taken.
City Attorney Cerwin informed the Committee that Ms. Fazen still needed to admit or
deny to the allegations in the complaint.
Ald. Roadt withdrew his motion and Ald. Weigel withdrew his second, that this matter
be Placed on File.
Discussion ensued regarding the allegations in the complaint. Ms. Fazen admitted to
the allegations.
The Committee agreed to dismiss this complaint based on Ms. Fazen had provided the
necessary documents.
A motion was made by Ald. Haass, seconded by Ald. May, that this matter was
Recommended to be Dismissed. The motion carried by the following vote:
Aye: 5- Ald. Czaplewski, Ald. Roadt, Ald. Haass, Ald. May, and Ald. Weigel
No: 0
CLOSED SESSION
A closed session was not held.
E. ADJOURNMENT
A motion was made by Ald. May, seconded by Ald. Haass, to adjourn at 6:55 p.m. The
motion carried unanimously.
