The meeting was called to order by Chair Czaplewski at 6:00 p.m.

A motion was made by Ald. Roadt, seconded by Ald. May, to Approve the Minutes (draft) of the
License & Health regular meeting of September 25, 2018, recess meetings of October 2, 2018
and October 16, 2018. The motion carried unanimously.
D. NEW AND PREVIOUS MATTERS
8. 2018-0820 Police Department Report involving Tomken's, 8001 W. Greenfield Ave., of
Tomken’s Inc., Michelle A. Felten, Agent, Incident Date: September 11, 2018.
Appearance by Michelle A. Felton, Gina M. Kowalik & Dave Wepking.
Discussion ensued regarding the event being held at Tomken's in the report. Ms.
Felton told the Committee they have been holding this charity event for several
years and the first time they held it they were told they would not need to get a
permit. Ms. Felton stated they were unaware of any permit changes. The

Committee informed Ms. Felton and Ms. Kowalik that for future events they
should contact the Clerk and Engineering Offices for proper permitting.
A motion was made by Ald. Weigel, seconded by Ald. Haass, that this matter was Recommended
to be Placed on File. The motion carried by the following vote:

2. 2018-0848 2018-2019 Combination Class A Liquor License application no. 2625, of State Fair
Liquor & Food Inc., Halder Pabitra, Agent, d/b/a State Fair Liquor & Food Mart,
for the premises located at 1568 S. 81 St. (new-existing location).
Nonappearance of Halder Pabitra. Mr. Pabitra did not contact the Clerk's Office.
The Committee requested he be ordered to appear at the next scheduled License &
Health meeting.

3. 2018-0813 2018-2019 Class B Tavern License application no. 2623, of La Costena Café, Hugo
Ramirez-Hernandez, Agent, d/b/a La Costena Café, for the premises located at 5823
W. Burnham St. (new-nonexisting location).
Appearance by Hugo Ramirez-Hernandez. Discussion ensued regarding the type
of cafe Mr. Ramirez-Hernandez was going to have and if he had experience.
Antonio Cayetano Rivera, from Las Palmas translated the Committee's questions
and comments to Mr. Ramirez-Hernandez. Mr. Ramirez-Hernandez was asked why
his alcohol sales were a different time than his hours of operation. Upon further
discussion Mr. Ramirez-Hernandez will change his hours of operation and
alcohol sales to 6:00 a.m. to 10:00 p.m.
A motion was made by Ald. Haass, seconded by Ald. May, that this matter was Recommended for
Approval subject to the necessary requirement(s). The motion carried by the following vote:

4. 2018-0787 2018-2019 Class B Beer License application no. 2624, of Escobar Kitchen LLC,
Esveiri Escobar, Agent, d/b/a Escobar’s Kitchen, for the premises located at 9130
W. Greenfield Ave. (new-nonexisting location).
Nonappearance of Esveiri Escobar. Ms. Escobar did not contact the Clerk's
Office. The Committee requested she be ordered to appear at the next scheduled
License & Health meeting.

5. 2018-0849 2018-2019 Operator’s License (bartender/sales clerk) application no. 19-528 of
Japhet M. Banda.
Appearance by Japhet M. Banda. Discussion ensued regarding Mr. Banda's
background check and his application.

Angela M. Crisci.
Second nonappearance of Angela M. Crisci. Ms. Crisci did not contact the
Clerk’s Office. The Committee Recommended denial based on two (2)
nonappearances.
A motion was made by Ald. Roadt, seconded by Ald. May, that this matter was Recommended for
Denial based on two (2) nonappearances. The motion carried by the following vote:

nonappearances.
A motion was made by Ald. May, seconded by Ald. Roadt, that this matter was Recommended for
Denial based on two (2) nonappearances. The motion carried by the following vote:

F. ADJOURNMENT
All meetings of the License & Health Committee are public meetings. In order for the general public to make comments at the committee
meetings, the individual(s) must be scheduled (as an appearance) with the chair of the committee or the appropriate staff contact; otherwise,
the meeting of the committee is a working session for the committee itself, and discussion by those in attendance is limited to committee
members, the mayor, other alderpersons, staff and others that may be a party to the matter being discussed.
