Weigel No: 0 41. 2019-0113 Petition to exceed the alcohol license quota submitted by Ronald Mellantine, for a 2018-2019 Class B Tavern Liquor License for Top Dog Enterprises, LLC, d/b/a Crawdaddy’s Roadhouse, located at 9638 W. National Ave. Attachments: Petition to exceed the Quota Appearance by Ronald Mellantine. A motion was made by Ald. May, seconded by Ald. Greenfield Ave., approved subject to necessary requirements on June 29, 2018, not yet issued as of February 14, 2019. Attachments: Pho Saigon letter Appearance by Ly Quian Vu and Attorney Bart D. Achterberg. A motion was made by Ald. Weigel, seconded by Ald. May, that this matter was Recommended to be Placed on File. The motion carried by the following vote: Aye: 5- Ald. Czaplewski, Ald. Roadt, Ald. Haass, Ald. May, and Ald. Weigel No: 0 43. 2019-0059 2018-2019 Operator’s License (bartender/sales clerk) application no. 19-566 of Shyanna N. Her. Nonappearance of Shyanna N. Her. Ms. Her did not contact the Clerk's Office. The Committee requested she be ordered to appear at the next scheduled License & Health meeting. This matter was Held 44. 2019-0058 2018-2019 Operator’s License (bartender/sales clerk) application no. 19-564 of Joseph J. Busalacchi, IV. Appearance by Joseph J. Busalacchi. The Committee requested Mr. Busalacchi to provide documentation of rehabilitation to the Clerk's Office and to appear at the meeting of March 5, 2019. This matter was Held 45. 2019-0060 2018-2020 Operator’s License (bartender/sales clerk) application no. 20-605 of Amanda J. Rumisek. Appearance by Amanda J. Rumisek. A motion was made by Ald.
