Roadt, seconded by Ald. Weigel, to Approve the minutes of the regular meeting of October 22, 2019, recess meetings of November 5, 2019 and November 19, 2019. The motion carried unanimously. D. NEW AND PREVIOUS MATTERS 2. 2019-0794 2019-2020 Adult-Oriented Establishment Operator Permit application of Christopher B Nieves Appearance by Christopher B Nieves. Discussion ensued regarding Mr. Nieves police report and his rehabilitation. A motion was made by Ald. May, seconded by Ald. Weigel, that this matter be Recommended For Approval based upon competent evidence of sufficient rehabilitation. The motion carried unanimously. Roadt, that this matter be Recommended For Approval based upon competent evidence of sufficient rehabilitation. The motion carried unanimously. 4. 2019-0788 2019-2021 Operator’s License (bartender/sales clerk) application of Kayla C Lustig Appearance by Kayla C Lustig. Discussion ensued regarding Ms. Lustig's police report and her rehabilitation. A motion was made by Ald. Weigel, seconded by Ald. May, that this matter be Recommended For Approval based upon competent evidence of sufficient rehabilitation. The motion carried unanimously. 5. 2019-0791 2019-2021 Operator’s License (bartender/sales clerk) application of Abigail KK Medcalf Appearance by Abigail KK Medcalf. Discussion ensued regarding Ms. Medcalf's police report and her rehabilitation. A motion was made by Ald. Weigel, seconded by Ald. May, that this matter be Recommended For Approval based upon competent evidence of sufficient rehabilitation. The motion carried unanimously. 6. 2019-0792 2019-2021 Operator’s License (bartender/sales clerk) application of Guadalupe Valdez, Jr. Appearance by Guadalupe Valdez, Jr. Discussion ensued regarding Mr. Valdez's police report, and his rehabilitation. The Committee requested additional information from Mr. Valdez to be submitted by Dececmber 3, 2019. This matter was Held. 7. 2019-0793 2019-2021 Operator’s License (bartender/sales clerk) application of Brandon L Welch Appearance by Brandon L Welch. Discussion ensued regarding Mr. Welch's police report and his rehabilitation. A motion was made by Ald. Weigel, seconded by Ald. May, that this matter be Recommended For Approval based upon competent evidence of sufficient rehabilitation. The motion carried unanimously. Ms. Lunsford did not respond to the letter mailed to her to allow her an appearance before the License & Health Committee to appeal the Committee's October 22, 2019 recommendation for denial based on the applicant's record having violations that are substantially related to the licensed activity. A motion was made by Ald. May, seconded by Ald. Weigel, that this matter be Recommended for Denial. Based on the record being substantially related to the licensed activity and the applicant's failure to provide competent evidence of rehabilitation. The motion carried unanimously. 9. 2019-0789 2019-2021 Operator’s License (bartender/sales clerk) application of Bridget M. McAdams Appearance by Bridget M. McAdams. Discussion ensued regarding Ms. Nieves police report and her rehabilitation. A motion was made by Ald. May, seconded by Ald. Roadt, that this matter be Recommended For Approval based upon competent evidence of sufficient rehabilitation. The motion carried unanimously. 10. 2019-0584 Police Department Report relative to the July 29, 2019 Alcohol Compliance Checks involving twelve (12) West Allis businesses, resulting with one (1) selling alcohol to an underage person without requesting identification (Layton Food & Gas LLC, Tarlok Bhatia, Agent, d/b/a Becher Liquor & Beer, premises located at 2077 S. 78 St) (Licensed Operator (bartender/sales clerk: Laura A. Saldivar) Appearance by Tarlok Bhatia. Discussion ensued regarding the compliance report and the steps Mr. Bhatia is taking to not let this occur again. A motion was made by Ald. Weigel, seconded by Ald. May, that this matter be Recommended to be Placed on File. The motion carried unanimously. 11. 2019-0752 Police Department Report relative to the September 8, 2019 Tavern compliance checks involving eleven (11) West Allis businesses, resulting in two (2) violations, Crawdaddy’s on Greenfield, LLC, Troy M. Meyer, Agent, d/b/a Crawdaddy’s on Greenfield, premises located at 9427 W. Greenfield Ave. Appearance by Troy Meyer. Discussion ensued regarding the compliance report and the steps Mr. Meyer is taking to not let this occur again. A motion was made by Ald. May, seconded by Ald. Roadt, that this matter be Recommended to be Placed on File. The motion carried unanimously. National Ave., of Stallywood LLC, Robert L. Anderson, Agent, Incident Date: July 4, 2019 Nonappearance of Robert L. Anderson. Mr. Anderson did not contact the Clerk’s Office. The Committee requested he be ordered to appear at the next scheduled License & Health meeting. This matter was Held. 13. 2019-0575 Police Department Report involving Red, White and Brews, 7127 W. National Ave., of Red, White and Brews, LLC, Eric G. Millard, Agent, Incident Date: July 4, 2019 Appearance by Eric G. Millard. Discussion ensued regarding the police report. A motion was made by Ald. Weigel, seconded by Ald. May, that this matter be Recommended to be Placed on File. The motion carried unanimously. 14. 2019-0746 Police Department Report involving Dingers, 6139 W. Beloit Rd., of Victory Hospitality Group, Steven P. Miick, Agent, Incident Date: September 18, 2019 Appearance by Steven P. Miick. Discussion ensued regarding the police report. A motion was made by Ald. May, seconded by Ald. Roadt, that this matter be Recommended to be Placed on File. The motion carried unanimously. 15. 2019-0748 Police Department Report involving Tap City U.S.A., 7207 W. National Ave., Tap City, Inc., Philip Kleist, Agent, Incident Date: September 20, 2019 Appearance by Philip Kleist. Discussion ensued regarding the police report. A motion was made by Ald. Weigel, seconded by Ald. Roadt, that this matter be Recommended to be Placed on File. The motion carried unanimously. 16. 2019-0742 Police Department Report involving Premier Sports Lounge, 1022 S. 60 St., of Premier Sports Lounge LLC, Tabitha A. Harris, Agent, Incident Date: September 6, 2019 Appearance by Tabitha A. Harris. Discussion ensued regarding the police report. This matter was Recommended to be Placed on File on a Block Vote. 17. 2019-0741 Police Department Report involving Broken Starr Saloon, 1100 S. 60 St., John Starr Pickles LLC, John R. Starr, Agent, Incident Date: September 6, 2019 Appearance by John R. Starr. Discussion ensued regarding the police report. This matter was Recommended to be Placed on File on a Block Vote. License & Health Committee recommended denial of the following applicants. The Clerk's Office was directed to send a letter allowing the applicant an opportunity to appear before the License & Health Committee to show competent evidence of rehabilitation. Balfe, James C. The motion carried unanimously. 19. 2019-0878 Discussion relative to general guidelines for reviewing license application background checks and the steps for approval or appearance Attachments: License Guidelines This matter was Held. 20. 2019-0885 Discussion on possible amendments to Chapter 9 related to imposing certain conditions on business operations of licenses This matter was Held.
