A motion was made by Ald. Roadt, seconded by Ald. Weigel, to Approve the minutes of the
regular meeting of October 22, 2019, recess meetings of November 5, 2019 and November 19,
2019. The motion carried unanimously.

Appearance by Kayla C Lustig. Discussion ensued regarding Ms. Lustig's police report and
her rehabilitation.

Approval based upon competent evidence of sufficient rehabilitation. The motion carried
unanimously.

Medcalf
Appearance by Abigail KK Medcalf. Discussion ensued regarding Ms. Medcalf's police
report and her rehabilitation.

Approval based upon competent evidence of sufficient rehabilitation. The motion carried
unanimously.
6. 2019-0792 2019-2021 Operator’s License (bartender/sales clerk) application of Guadalupe
Valdez, Jr.
Appearance by Guadalupe Valdez, Jr. Discussion ensued regarding Mr. Valdez's police
report, and his rehabilitation. The Committee requested additional information from Mr.
Valdez to be submitted by Dececmber 3, 2019.

Welch
Appearance by Brandon L Welch. Discussion ensued regarding Mr. Welch's police report
and his rehabilitation.

Approval based upon competent evidence of sufficient rehabilitation. The motion carried

8. 2019-0787 2019-2021 Operator’s License (bartender/sales clerk) application of Shelby L
Lunsford
Nonappearance of Shelby L Lunsford. Ms. Lunsford did not respond to the letter mailed
to her to allow her an appearance before the License & Health Committee to appeal the
Committee's October 22, 2019 recommendation for denial based on the applicant's
record having violations that are substantially related to the licensed activity.
A motion was made by Ald. May, seconded by Ald. Weigel, that this matter be Recommended for
Denial. Based on the record being substantially related to the licensed activity and the
applicant's failure to provide competent evidence of rehabilitation. The motion carried
unanimously.
9. 2019-0789 2019-2021 Operator’s License (bartender/sales clerk) application of Bridget M.
McAdams
Appearance by Bridget M. McAdams. Discussion ensued regarding Ms. Nieves police report
and her rehabilitation.
A motion was made by Ald. May, seconded by Ald. Roadt, that this matter be Recommended For
Approval based upon competent evidence of sufficient rehabilitation. The motion carried
unanimously.
10. 2019-0584 Police Department Report relative to the July 29, 2019 Alcohol Compliance Checks
involving twelve (12) West Allis businesses, resulting with one (1) selling alcohol to an
underage person without requesting identification (Layton Food & Gas LLC, Tarlok
Bhatia, Agent, d/b/a Becher Liquor & Beer, premises located at 2077 S. 78 St)
(Licensed Operator (bartender/sales clerk: Laura A. Saldivar)
Appearance by Tarlok Bhatia. Discussion ensued regarding the compliance report and
the steps Mr. Bhatia is taking to not let this occur again.
A motion was made by Ald. Weigel, seconded by Ald. May, that this matter be Recommended to
be Placed on File. The motion carried unanimously.
11. 2019-0752 Police Department Report relative to the September 8, 2019 Tavern compliance
checks involving eleven (11) West Allis businesses, resulting in two (2) violations,
Crawdaddy’s on Greenfield, LLC, Troy M. Meyer, Agent, d/b/a Crawdaddy’s on
Greenfield, premises located at 9427 W. Greenfield Ave.
Appearance by Troy Meyer. Discussion ensued regarding the compliance report and the
steps Mr. Meyer is taking to not let this occur again.

Health meeting.
