Dee Williams shared the Derby High School Adopt a Panther picture of Tylor Lovette. RE: MySeniorCenter sign in. Dee Williams reminded Board members to record their volunteer hours. Kim Hart will do a demonstration at the kiosk for the Board members after the meeting. RE: Medicare Plan D. Jim Burgess stated Medicare Plan D appointments start next week on October 15th. Agenda Item #2 RE: Advisory Board Fund report for September 2015. Diane Schmitt gave the Advisory Board Fund report for September. The deposit total was $855.00, expenditures were $725.91 with an ending balance $13,471.48. MOTION: Gerald Brownlee moved to receive and file the Advisory Board Fund report for September. Tom Davidson seconded. VOTE: 5 yea, motion carried. Agenda Item #3 RE: Approve September 2, 2015 Advisory Board Meeting minutes. MOTION: Jim Burgess made a motion to approve the minutes from September 2, 2015. Diane Schmitt seconded. 1 VOTE: 5 yea, motion carried. Agenda Item #4 RE: Approve September 2015 Derby Dash Scholarship Report. Dee Williams gave the report for the September Derby Dash Scholarship Report. There was $40 used for scholarships in September, leaving an ending balance $344.33. Dee let the Board know she filled out an application for Wal-Mart to request funds for Derby Dash Scholarship. MOTION: Janice Neagle made a motion to approve the Derby Dash Scholarship Report for September 2015. Tom Davidson seconded. VOTE: 5 yea, motion carried. Agenda Item #5 RE: Accreditation. Dee Williams stated that the Action Plan 1, 2 & 3 need to be approved with revisions by the Advisory Board. MOTION: Jim Burges made a motion to approve the Action Plan 1, 2 & 3 as revised. Tom Davidson seconded. VOTE: 5 yea, motion carried. Agenda Item #6 RE: September TNT. Tom Davidson said they served Baked Ham Slices with a total profit of $269.67. Tom said they fed a total of 103. Agenda Item #7 RE: Quilt Ticket Sales. Kim Hart stated the total amount for Quilt ticket sales in September was $190. Janice Neagle is the Board member responsible for the October Quilt ticket sales.
